The development and feasibility study of evidence-based nursing care for promoting peripheral tissue perfusion among critically ill patients with peripheral artery disease

Abstract

The study aimed to develop and evaluate the feasibility of evidence-based nursing care for promoting peripheral tissue. The center of advanced nursing practice evidence-based practice model was adopted in evidence-based nursing care development. Quality was measured in terms of content validity index (CVI) and Appraisal of Guidelines for Research & Evaluation II (AGREE II), which were verified by five experts. The evidence-based nursing care was tried out in 10 patients to evaluate the feasibility by assessing the ABI level and signs of 6P after 24 hours of care. Data were analyzed using descriptive statistics, Paired t-test and Chi-square.

 The final searching and synthesis included a total of 18 articles to develop evidence-based nursing care. This evidence-based nursing care is composed of 3 main parts, including 1) assessment of signs and symptoms of peripheral tissue perfusion, 2) laboratory tests, and 3) nursing care for promoting peripheral tissue perfusion. Evidence-based nursing care has CVI= 1.00, AGREE II=93.57-100.00 percent, and 5 experts recommended applying this evidence-based nursing care in patients with peripheral artery disease.

 The results of the study after 24 hours of care demonstrated that all patients had an ankle-brachial index level (mean=0.95, SD=0.30) higher than the pre-intervention (mean=0.71, SD=0.20) with statistical significance (t=5.451, p<0.001). The incidence rate signs of 6Ps were lower than the pre-intervention but they were not different from the pre-intervention with statistical significance (p<0.05). The pain score level (mean=2.90, SD=1.63) was lower than the pre-intervention (mean=8.50, SD=1.65) with statistical significance (t=10.340, p<0.001).

This evidence-based nursing care has adequate quality, and it can help to increase peripheral tissue perfusion in patients with peripheral artery disease. Future research should be carried out to evaluate the effectiveness of evidence-based nursing care.
