Development of a network of caregivers for home-bound and bedridden older adults to prevent depression

The present study was a participatory action research (PAR) with the objective of creating a network of caregivers for home-bound and bedridden older adults to prevent depression. The research process consisted of 4 phases: 1) Study the situation of depression and care needs of home-bound and bedridden older adults; 2) Create a network of caregivers for home-bound and bedridden older adults to prevent depression; 3) Develop potential of the network of caregivers for homebound and bedridden older adults to prevent depression; and 4) Conduct home visits and evaluate the network of caregivers for homebound and bedridden older adults.
The sample group consisted of 19 home-bound older adults; 25 bedridden older adults; 17 people form community/local service networks; and 30 people form networks of caregivers for home-bound and bedridden older adults. Research tools consisted of 1) 2Q and 9Q Thai version Patient Health Questionnaire depression screening tools (PHQ-2 and PHQ-9), 2) a home visit record form, 3) a satisfaction questionnaire for families of home-bound and bedridden older adults. Quantitative data were analyzed using descriptive statistics and paired t-test statistics. Qualitative data were analyzed using content analysis.
The research findings revealed that most homebound and bedridden older adults (50.30%) had no symptom/very low levels of depression. Components of the older adult care network included: 1) The main care network, comprising nursing staff at the Subdistrict Health Promoting Hospital, Village Health Volunteers, and volunteers; 2) support network, consisting of local administrative organizations and village leaders. The results of developing the potential of the network of older adult caregivers found that older adult caregivers’ network had statistically significant higher knowledge and skills in performing home visits and using a set of 5anti-depressant activities; including 1) depression assessment, 2) relaxation massage, 3) meditation therapy, 4) therapeutic conversation, and 5) Mor Lam (northeastern Thai-style) music therapy; than before training (p<0.001).
The results of this study showed that a network of caregivers for home-bound and bedridden older adults had been created. The developed network could assist in caring mental health for older adults, by providing knowledge and organizing activities to promote mental health among older adults, leading to be able to prevent depression.
