การวิจัยเชิงปฏิบัติการมีวัตถุประสงค์เพื่อพัฒนารูปแบบการฟื้นฟูและดูแลผู้สูงอายุภาวะสมองเสื่อมโดยการมีส่วนร่วมของชุมชน ผู้ร่วมวิจัย จำนวน 46 คน คือ ผู้มีส่วนได้ส่วนเสียในชุมชน ที่คัดเลือกแบบเจาะจงตามเกณฑ์คุณสมบัติ รวบรวมข้อมูลเชิงคุณภาพด้วยการสัมภาษณ์เชิงลึกและสนทนากลุ่ม วิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา การศึกษา มี 3 ระยะ คือ การวิเคราะห์สถานการณ์ กระบวนการพัฒนาและการสังเคราะห์รูปแบบ ผลการวิจัยได้ รูปแบบ “ท่าสองคอนโมเดล" มี 4 องค์ประกอบสำคัญ ได้แก่ นโยบาย บุคลากร การบริการและระบบสนับสนุน และมี 3 กิจกรรมบริการ ได้แก่ การประเมินและคัดกรอง การส่งต่อเพื่อรักษาและการบริการฟื้นฟูและดูแลโดยการมีส่วนร่วมของชุมชน รูปแบบการบริการนี้เหมาะสำหรับการให้บริการผู้สูงอายุภาวะสมองเสื่อมในบริบทของการดูแลผู้สูงอายุในชุมชน
