การสร้างพลังอำานาจกับการจัดการโรคเรื้อรังในเขตชนบท (Empowerment and Chronic Illness in a Rural Context)

Abstract

การจัดการดูแลผู้เจ็บป่วยด้วยโรคเรื้อรังที่มีประสิทธิภาพเพื่อให้บุคคลสามารถดำารงไว้ซึ่งหน้าที่และมีภาวะสุขภาพที่ดีที่สุด ต้องมีความสอดคล้องกับวิถีชีวิต อุปสรรคสำาคัญของการจัดการดูแลผู้เจ็บป่วยด้วยโรคเรื้อรังในพื้นที่ห่างไกลในเขตชนบท คือ การขาดแคลนบุคลากรทางสุขภาพ และทรัพยากรเพื่อสนับสนุนการจัดการดูแลสุขภาพ การใช้แนวคิดการสร้างพลังอำานาจในการจัดการบริการที่อาศัยการจัดการบริการในลักษณะผู้ใช้บริการเป็นศูนย์กลาง และการจัดการดูแลอย่างต่อเนื่อง น่าจะเป็นแนวทางที่น่าจะช่วยให้ผู้ใช้บริการสามารถควบคุมการดำาเนินชีวิต มีอิสระ และจัดการดูแลสุขภาพของตนเอง การส่งเสริมการมีส่วนร่วมของบุคคล ครอบครัว และชุมชนเพื่อเป็นเครือข่ายในการดูแลโรคเรื้อรัง เป็นวิธีการที่จะทำาให้ทรัพยากรในการจัดการดูแลโรคเรื้อรังเพิ่มมากขึ้นในพื้นที่ห่างไกลในทางอ้อม ส่งผลดีต่อภาวะสุขภาพ และความผาสุกของผู้เจ็บป่วยด้วยโรคเรื้อรัง ดังนั้น พยาบาลจึงควรนำาแนวคิดเรื่องการสร้างพลังอำานาจมาใช้ทั้งในการจัดการศึกษาพยาบาล การวิจัย และการปฏิบัติการพยาบาล เพื่อพัฒนาการจัดการดูแลสำาหรับโรคเรื้อรังต่อไป

Integration of clients’ life style into care and illness management is key to effective chronic illness management with the aim to maintain clients’ functioning and to support the clients to gain a maximum level of well-being. The shortage of health professionals and health care resources represent significant barriers to illness management. Health care based on empowerment through client centredness and continuing care is likely to enable people with chronic conditions to control their management freely. Indirectly, health care resources in rural areas can be increased through the involvement of family and community in care. These benefits of empowerment strategies support those living with chronic disease to achieve better health outcomes and well-being. Nursing education, research and practice using empowerment concepts as a foundation may improve health care for people living with chronic illness.
