การประกอบสร้างทางสังคมของบิดามารดา ในการแสดงออกถึงความปวดของเด็กในบริบทอีสาน (Social Construction of Parents on Pediatric Pain Expression in the Context of Isan)

การวิจัยเชิงคุณภาพนี้มีวัตถุประสงค์เพื่อศึกษาความคิดความเชื่อของบิดามารดาในการเลี้ยงดูเด็กตามเพศภาวะในชีวิตประจำาวัน และ เมื่อเผชิญกับความปวดด้วยการวิเคราะห์เชื่อมโยงกับแนวคิดทางเพศภาวะ (gender-based analysis) ผู้ให้ข้อมูลเป็นบิดามารดาหรือครอบครัวของเด็กรับการผ่าตัดอายุ 3-18 ปี ที่นอนรักษาตัวในโรงพยาบาล 3 แห่งในภาคตะวันออกเฉียงเหนือจำานวน 20 ครอบครัว และมีผู้ให้ข้อมูลทั่วไป 14 คน ผลการวิจัยพบแก่นความคิดของการประกอบสร้างทางสังคมและการขัดเกลาทางสังคม (social construction and socialization) 8 แก่นความคิดดังนี้ 1) ผู้หญิงสูงวัย: ผู้สืบทอดพิธีกรรมความเชื่อ 2) ผู้หญิงอ่อนหวานบริการ ผู้ชายผู้สร้างผู้นำาตามคติอีสาน 3) ลูกหญิง ต้องรู้อยู่ ลูกชายไปไหนไปได้: ผลของการขัดเกลาทางสังคมตามบทบาททางเพศ 4) ผลพวงของการเลี้ยงดูแบบชาย เป็นใหญ่: พ่อแม่ลำาเอียง 5) ผลพวงของการแสดงบทบาททางเพศเมื่อลูกเจ็บป่วย: แม่คือผู้ดูแลที่ทุ่มเท 6) การขัดเกลาลูกตามบทบาททางเพศเมื่อลูกปวด: ลูกชายลูกหญิงต่างกัน 7) ผลลัพธ์การแสดงออกซึ่งความปวด: เด็กหญิงเด็กชายแตกต่าง และ 8) จากอคติทางเพศตามเพศภาวะทางสังคม: สู่อคติทางเพศของบุคลากรสุขภาพต่อผู้ป่วยเด็ก ข้อค้นพบนี้ ได้สร้างข้อเสนอแนะเชิงนโยบายการจัดการความปวดในเด็กให้มีความละเอียดอ่อนเชิงเพศภาวะ (gender-sensitive pediatric pain management)
This qualitative study with gender-based analysis investigated the beliefs of parents in daily life child rearing and child pain expression. Informants were composed of 20 operative children aged 3 to 18 years with the families admitted at three hospitals in Northeastern Thailand, and 14 general informants. Findings revealed 8 major themes related to social construction and family socialization including; 1) Grandmothers function as mediators of beliefs and rituals; 2) Isan cultural regulation on boys’ and girls’ performances; 3) Boys have more free time than girls: the results of gender socialization; 4) Men are always important in the families in patriarchal society: It is unfair; 5) Mothers are the persons who put their soul in taking care of sick children: Gender stereotypes; 6) Gender role socialization of children pain expression: doing differences in sons and daughters; 7) Outcomes of pediatric pain expression: girls and boys are different; and 8) From gender stereotype bias to gender bias in pediatric pain management of health care providers. The research findings suggest that the policy of gender-sensitive pediatric pain management should be concerned and developed.
