Development of Care Model On Sepsis Patients Pathumrat Hospital

Purposes : To study situation, develop a model, and study on results of model for care model on sepsis patients, Pathumrat Hospital.
Study design : Research and Development design.
Materials and Methods : 16 of sample consisted were professional nurses who were users of guidelines and studied 68 of medical records of patients diagnosed with sepsis by doctors. The research were conducted into 3 phases: 1) analyze situation by reviewing medical records and reviewing patient care guidelines according to sepsis guideline 2) developing a patient care system by training workshops for nurses to put them into practice, and 3) evaluate results. Data were collected using recording forms, assessment forms, and knowledge tests, and data were analyzed using frequency statistics, percentages, means, standard deviations, and Student's t-tests. Statistical significance was set at 95% confidence Interval.
Main findings : The model for caring for patients with sepsis patients consists of the process of (1) screening patients at risk for sepsis patients upon initial admission (2) caring for patients with sepsis and (3) transferring patients with sepsis patients. And results of operations found that results of reviewing medical records in 2022, 34 cases, in 2023, 34 cases, found that adequate fluid replacement was given, accounting for 91.17%, and 100% sent for culture before administering medication. Antibiotics were 76.47% and 97.05%, respectively. Receiving antibiotics within 1 hour after diagnosis were 88.23% and 100%. The mean nursing practice scores according to nurses' guidelines increased from before and nurses' mean knowledge scores increased (p< .001).
Conclusion and recommendations : Improving care for patients with sepsis patients infections from process of using nursing guidelines, patients with sepsis were assessed and critically monitored. resulting in better care results.
