The Development of Triage Patients Model by Emergency Nurses in Emergency Department at Kaset wisai Hospital

Abstract

Purposes : To study the problems and needs, create and check quality and trial, and evaluation of patient triage model by nurses.

Study design : Research and development approach.

Materials and Methods : Nine of nurses target group were consisted and 28 of admitted patients who were selected purposive sampling. We were conducted research from January - August 2023. A process research were four steps, studying of  problems and needs, developed model, Implementation, and evaluation. Data were collected using tests, assessments, and group discussion questions. Data were analyzed using frequency, percentage, mean, and standard deviation, and compare the mean knowledge scores regarding patient sorting using Percentage differences.

Main findings : Found that  problems and needs in patient sorting among emergency department nurses in (1) the entire number of working personnel competency and skill development (2) process, location and (3) separation equipment,  The triage model consisted of seven steps: (1) Assessing symptoms to separate patients (Primary Assessment), (2) Classifying and prioritizing urgency, (3) Diagnosing and providing medical treatment according to guidelines, (4) Providing first aid in dangerous crisis situations and risk of death according to guidelines (5) Report to doctor or care team (6) Assess and separate respiratory diseases and separate zones waiting to be examined, and (7) Record the preliminary separation. And nurses' knowledge scores after training increased (89.63%) and were able to use patient triage tools correctly by (92.52%). Results of patient triage decreased with errors. It was found that the assessment of the level of severe over triage (0.18%) (38/211) and severe under triage (0.56%) (58/103).

Conclusion and recommendations : The results of this research result in a reduction in Under Triage/over Triage. Therefore, accident and emergency departments should use them to develop their work.
