The Development of an Emergency Room Triage Model On Triage Nurse Frontier point Thawatburi Hospital

Abstract

Purpose : To study problems and needs assessment, development, and evaluate of an emergency room triage model on triage nurse frontier point Thawatburi Hospital.

Study design : Participative action research (Action Research: AR)

Materials and Methods : 24 of samples consisted the physicians and nurses working at Emergency accident Unit and 510 of persons receiving services at emergency accident agency, conducted from July - September 2022. Data were collected using questionnaires, in-depth interview group, focus group discussion, and assess satisfaction. The data were analyzed using a frequency, percentage, mean, standard deviation, percentage differential and Wilcoxon Signed Ranks Test.

Main findings : 1) The problem condition and  need for patient screening,  the cause was caused by experience, knowledge and skills fatigue and segregation guidelines service recipients had urgent service needs, change symptoms unclear symptoms many emergency patients receive services at same time during same period, and patients without relatives could not take a history or unclear history, 2) development of model were consisting of (1) All general medical examination patients who received services must be screened at the screening point. All cases were registered nurses. (2) Patients were sorted by using the emergency severity Index of 5 levels, sorted from categories 1,2, and level 3 to 2, and sent to the emergency room. (3) Level 3, 4 and 5 patients sent to go to outpatient department and 3) after the development, it was found that mean under triage decreased by 25.00% and over triage decreased by 18.50% and mean waiting time for the doctor to examine the most was reduced, which was the waiting time for the ESI5 doctor = 9.07% and the practitioner. The work was satisfied with the differentiation of emergency patients (p<.001).

Conclusion and recommendations : A patient screening system should be developed using five levels of emergency and an experienced nurse should be present at the isolation point.
