บทคัดย่อ

การถ่ายโอนภารกิจด้านสาธารณสุขสู่องค์การบริหารส่วนจังหวัด ส่งผลให้ผู้อำนวยการกองสาธารณสุข มีบทบาทสำคัญมากขึ้นในการบริหารจัดการระบบสุขภาพปฐมภูมิระดับพื้นที่ การวิจัยเชิงคุณภาพแบบพรรณนา ครั้งนี้มีวัตถุประสงค์เพื่อวิเคราะห์สถานการณ์การบริหารจัดการระบบบริการสุขภาพปฐมภูมิของผู้อำนวยการกองสาธารณสุขในองค์การบริหารส่วนจังหวัด ภายหลังการถ่ายโอนภารกิจจากกระทรวงสาธารณสุข พื้นที่ศึกษาครอบคลุม 8 จังหวัด จาก 4 ภูมิภาค ผู้ให้ข้อมูลรวม 256 คน ประกอบด้วย ผู้กำหนดนโยบาย ผู้ปฏิบัติงาน บุคลากรหน่วยบริการปฐมภูมิที่ถ่ายโอน และผู้แทนภาคประชาชน โดยมีผู้อำนวยการกองสาธารณสุขเป็นผู้ให้ข้อมูลหลัก เครื่องมือที่ใช้ประกอบด้วย ผู้วิจัย แบบสัมภาษณ์กึ่งโครงสร้าง แนวทางสนทนากลุ่มและแบบบันทึกข้อมูลที่เกี่ยวข้อง เก็บข้อมูลโดยการสัมภาษณ์เชิงลึก การสนทนากลุ่ม และการทบทวนเอกสาร วิเคราะห์ด้วยการวิเคราะห์เนื้อหา ตรวจสอบความน่าเชื่อถือด้วยการตรวจสอบแบบสามเส้า การสะท้อนกลับข้อมูล และการตรวจสอบโดยผู้เชี่ยวชาญ ผลการวิจัย พบว่า ภายหลังการถ่ายโอน บทบาทของผู้อำนวยการกองสาธารณสุขเปลี่ยนจากผู้บริหารงานสาธารณสุขทั่วไปสู่ “ผู้จัดการระบบ” ที่ต้องเชื่อมโยงการทำงานระหว่างหน่วยบริการ หน่วยงานกำกับ และภาคีในระบบสุขภาพ สรุปได้ 5 ประเด็น ได้แก่ (1) การปรับโครงสร้างและระบบบริหารให้สอดคล้องกับบริบทพื้นที่ (2) การบริหารทรัพยากรมนุษย์และการพัฒนาสมรรถนะบุคลากร (3) ความท้าทายด้านมาตรฐานบริการและระบบกำกับคุณภาพ (4) การประสานงานเครือข่ายและกลไกการอภิบาล และ (5) การจัดระบบสุขภาพปฐมภูมิในฐานะเครือข่ายภายใต้ระบบหลักประกันสุขภาพ ข้อเสนอเชิงนโยบาย ได้แก่ การพัฒนากลไกกำกับดูแลและระบบข้อมูลสุขภาพที่ชัดเจน การเสริมสร้างสมรรถนะด้านการอภิบาลระบบสุขภาพของผู้อำนวยการกองสาธารณสุข และการสร้างความร่วมมือระหว่างหน่วยงานในระดับพื้นที่ เพื่อให้บริการสุขภาพปฐมภูมิมีคุณภาพ ต่อเนื่อง และประชาชนเข้าถึงบริการได้อย่างเสมอภาค
