บทคัดย่อ

มะเร็งเต้านมถือเป็นมะเร็งที่มีอุบัติการณ์เกิดสูงเป็นอันดับหนึ่งในสตรีทั่วโลกและมีแนวโน้มเพิ่มขึ้นในกลุ่มสตรีสูงอายุ การตรวจหามะเร็งเต้านมโดยการตรวจเต้านมโดยบุคลากรทางการแพทย์มีส่วนสำคัญในการเพิ่มโอกาสในการรักษาได้ การวิจัยเชิงพรรณนาหาความสัมพันธ์เชิงทำนายมีวัตถุประสงค์เพื่อ 1) ความตั้งใจในการเข้ารับการตรวจเต้านมโดยบุคลากรทางการแพทย์ของสตรีสูงอายุ และ 2) ปัจจัยที่ทำนายความตั้งใจในการเข้ารับการตรวจเต้านมโดยบุคลากรทางการแพทย์ของสตรีสูงอายุ กลุ่มตัวอย่าง คือ สตรีสูงอายุตั้งแต่ 60–69 ปี ที่อาศัยอยู่ในจังหวัดเชียงใหม่จำนวน 200 คน เก็บรวบรวมข้อมูลโดยใช้แบบสอบถามระหว่างเดือนมกราคม ถึง เดือนมีนาคม 2568 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและสถิติถดถอยโลจิสติกส์ กำหนดค่านัยสำคัญทางสถิติ (p<.05) ผลการศึกษาพบว่า สตรีสูงอายุมีค่าคะแนนเฉลี่ยความตั้งใจเข้ารับการตรวจเต้านมโดยบุคลากรทางการแพทย์ เท่ากับ 74.48 คะแนน (SD=25.03) ตัวแปรที่มีอำนาจในการทำนายความตั้งใจในการเข้ารับการตรวจเต้านมโดยบุคลากรทางการแพทย์สูงสุดคือ การรับรู้อุปสรรคของการตรวจคัดกรองมะเร็งเต้านม (OR=0.92; 95% CI, 0.88-0.96) รองลงมาคือการรับรู้ความเสี่ยงของการเป็นมะเร็งเต้านม (OR=1.06; 95% CI, 1.00-1.13) โดยทั้งสองตัวแปรสามารถร่วมกันทำนายความตั้งใจของสตรีสูงอายุในการเข้ารับการตรวจเต้านมโดยบุคลากรทางการแพทย์ ได้ร้อยละ 15.4 (R²=0.154, p<.001) การวิจัยในครั้งนี้สามารถนำข้อมูลพื้นฐานในการออกแบบโปรแกรมการคัดกรองมะเร็งเต้านม ในสตรีสูงอายุโดยมุ่งเน้นการลดอุปสรรคด้านความรู้และความเข้าใจเกี่ยวกับการตรวจเต้านม เพื่อส่งเสริมให้สตรีสูงอายุเข้ารับการตรวจเต้านมโดยบุคลากรทางการแพทย์อย่างต่อเนื่องและสม่ำเสมอ
