การพัฒนารูปแบบการบริบาลทางเภสัชกรรมที่บ้าน สำหรับผู้ป่วยเบาหวานที่มีภาวะแทรกซ้อนโรคไตเรื้อรัง ในเขตตำบลหนองแสง อำเภอวาปีปทุม จังหวัดมหาสารคาม Development of Home Pharmaceutical Care Model for Diabetes Mellitus Patients with Chronic Kidney Disease at Nong

Abstract

การวิจัยเชิงปฏิบัติการครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาการพัฒนารูปแบบการบริบาลทางเภสัชกรรมที่บ้านสำหรับผู้ป่วยเบาหวานที่มีภาวะแทรกซ้อนโรคไตเรื้อรังในเขตตำบลหนองแสง อำเภอวาปีปทุม จังหวัดมหาสารคาม โดยกลุ่มเป้าหมายประกอบด้วยกลุ่มผู้ที่มีส่วนเกี่ยวข้องกับการพัฒนารูปแบบ จำนวน 37 คน และผู้ป่วยกลุ่มตัวอย่าง จำนวน 56 คน ดำเนินการวิจัยทั้งโดยการเก็บรวบรวมข้อมูลเชิงปริมาณและข้อมูลเชิงคุณภาพ โดยข้อมูลเชิงปริมาณใช้แบบทดสอบความรู้ แบบบันทึกข้อมูลผู้ป่วย แบบประเมินความพึงพอใจ และแบบประเมินมาตรฐานงานเภสัชกรรมปฐมภูมิ ข้อมูลเชิงคุณภาพโดยการระดมสมองและการถอดบทเรียน วิเคราะห์ข้อมูลเชิงปริมาณใช้สถิติความถี่  ร้อยละ ค่าเฉลี่ยและส่วนเบี่ยงเบนมาตรฐาน การวิเคราะห์ข้อมูลเชิงคุณภาพโดยการวิเคราะห์เชิงเนื้อหาผลการศึกษา พบว่า การพัฒนารูปแบบการบริบาลทางเภสัชกรรมที่บ้านสำหรับผู้ป่วยเบาหวานที่มีภาวะแทรกซ้อนโรคไตเรื้อรังในเขตตำบลหนองแสง อำเภอวาปีปทุม จังหวัดมหาสารคาม ประกอบด้วย    9 กิจกรรม คือ 1) การประชุมกลุ่มย่อย 2) การแต่งตั้งคณะทำงาน 3) การจัดทำแผนปฏิบัติการ 4) การพัฒนาศักยภาพด้านรูปแบบ 5) การบริบาลทาง    เภสัชกรรม ที่บ้านเพื่อค้นหาและแก้ไขปัญหาการใช้ยา 6) การบริบาลทางเภสัชกรรมที่บ้านเพื่อติดตามการแก้ไขปัญหาการใช้ยา 7) การประเมินความพึงพอใจ   8) การประเมินผลการดำเนินงานเภสัชกรรมปฐมภูมิ และ 9) การถอดบทเรียน ซึ่งจะส่งผลให้ผู้ป่วยมีความรู้เกี่ยวกับยา ให้ความร่วมมือในการรักษาและผู้ป่วยมีคุณภาพชีวิตที่ดีขึ้น สามารถแก้ไขปัญหาการใช้ยาได้ร้อยละ 100 กลุ่มผู้ป่วยมีความพึงพอใจมากที่สุดต่อการบริบาลทางเภสัชกรรมที่บ้านและผลการประเมินการดำเนินงานของหน่วยบริการปฐมภูมิผ่านตามเกณฑ์มาตรฐานงานเภสัชกรรมปฐมภูมิ โดยสรุป ปัจจัยแห่งความสำเร็จในครั้งนี้ ประกอบด้วย ความร่วมมือในกระบวนการเยี่ยมบ้าน ประสบการณ์และความรู้ของทีมเยี่ยมบ้าน รวมทั้งความพร้อมรับการเปลี่ยนแปลงในการบริบาลทางเภสัชกรรมที่บ้าน

              This action research was aimed to study the development of home pharmaceutical care model for diabetes mellitus patients with chronic kidney disease at Nong Saeng Sub-district, Wapipathum District, Mahasarakham Province. Total sample group comprised of fifty-six patients and thirty-seven medical professional   participants. The research was quantitative and qualitative analysis with the tools of the quantitative data and knowledge test form, patient profile form, satisfaction evaluation form and standard form of primary pharmaceutical care service. Qualitative data was collected by using participant brainstorming and lesson learned methods. The quantitative statistics were frequency, percentage, means and standard deviation. Qualitative data was analyzed through content analysis. The results revealed nine activities of the development process at Nong Saeng Sub-district, Wapipathum District, Mahasarakham Province which were 1) focus group discussion 2) assignment of working group 3) action plan 4) potential development of the model 5) home pharmaceutical care related to drug used and solving problems    6) following up home pharmaceutical care for drug used problem solving 7) satisfaction evaluation 8) evaluation of standard criteria performance 9) lesson learned method. Therefore the patients can obtain the improvement in knowledge of medication adherence, quality of life and decrease 100 % of drug related problems. The patients are highly satisfied with this home pharmaceutical care which can achieve the standard operating criteria of primary pharmaceutical care service. In conclusion, the Key success factors consist of cooperation in process of home care, experience and knowledge of home care team and readiness to the change of the home pharmaceutical care.
