ผลของโปรแกรมการออกกำลังกายต่ออาการผิดปกติทางระบบประสาทส่วนปลายจากเคมีบำบัดในผู้ป่วยโรคมะเร็งลำไส้ใหญ่ และทวารหนักที่ได้รับยาออกซาลิพลาทิน

อาการผิดปกติทางระบบประสาทส่วนปลายจากยาเคมีบำบัด (Chemotherapy-Induced Peripheral Neuropathy: CIPN) เป็นอาการข้างเคียงที่พบบ่อยในผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนักที่ได้รับยาออกซาลิพลาทิน ส่งผลกระทบต่อการทำหน้าที่ของร่างกายและคุณภาพชีวิต การออกกำลังกายแบบผสมผสานเป็นแนวทางหนึ่งที่มีศักยภาพในการป้องกันและลดความรุนแรงของอาการดังกล่าว การวิจัยกึ่งทดลองแบบสองกลุ่ม วัดผลก่อนและหลังนี้ มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมการออกกำลังกายต่ออาการผิดปกติทางระบบประสาทส่วนปลายจากเคมีบำบัดในผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนักที่ได้รับยาออกซาลิพลาทิน กลุ่มตัวอย่างจำนวน 50 คน คัดเลือกจากผู้ป่วยอายุ 20 ปีขึ้นไป ที่ได้รับยาเคมีบำบัดสูตรซีลอก ณ หน่วยเคมีบำบัดผู้ป่วยนอกโรงพยาบาลศรีสะเกษ โดยใช้วิธีจับคู่อายุและรอบของยาเคมีบำบัด แบ่งเป็นกลุ่มทดลองที่ได้รับโปรแกรมการออกกำลังกายแบบผสมผสาน และกลุ่มควบคุมที่ได้รับการพยาบาลตามปกติ เครื่องมือวิจัยประกอบด้วยแบบสอบถามข้อมูลส่วนบุคคล แบบประเมิน EORTC QLQ-CIPN20 โปรแกรมและสื่อการออกกำลังกาย ซึ่งผ่านการตรวจสอบคุณภาพแล้ว วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา Mann-Whitney U test และ Wilcoxon signed-rank test
ผลการศึกษาพบว่ากลุ่มทดลองมีความรุนแรงของอาการ CIPN ด้านการรับความรู้สึกและการเคลื่อนไหวต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p<.001) และหลังได้รับโปรแกรมการออกกำลังกาย อาการผิดปกติด้านการเคลื่อนไหวลดลง อย่างมีนัยสำคัญ (p=.031) แต่ด้านการเคลื่อนไหวและประสาทอัตโนมัติไม่แตกต่าง ผลการวิจัยชี้ให้เห็นว่าโปรแกรมการออกกำลังกายแบบผสมผสานมีประสิทธิผลในการลดอาการ CIPN โดยเฉพาะด้านการเคลื่อนไหวในผู้ป่วยกลุ่มนี้
