ประสิทธิผลของโปรแกรมการดูแลต่อเนื่องต่อความพร้อมก่อนออกจาก โรงพยาบาลในผู้ป่วยศัลยกรรม-อายุรกรรมในเอเชีย : การทบทวนวรรณกรรมอย่างเป็นระบบ

บทคัดย่อ

การศึกษานี้เป็นการทบทวนวรรณกรรมอย่างเป็นระบบ และวิเคราะห์ผลรวมเชิงสถิติ เพื่อประเมินประสิทธิผลของโปรแกรมการดูแลต่อเนื่อง ต่อความพร้อมก่อนออกจากโรงพยาบาล (Readiness for Hospital Discharge) และอัตราการกลับเข้ารักษาซ้ำ (Hospital Readmission) ในผู้ป่วยศัลยกรรมและอายุรกรรมในเอเชีย งานวิจัยที่ใช้ในการวิเคราะห์ถูกคัดเลือกจากฐานข้อมูลทางวิชาการที่ได้รับการยอมรับในระดับสากล ได้แก่ PubMed, Scopus, Web of Science, CINAHL, Cochrane Library, Embase, ScienceDirect และฐานข้อมูลวิทยานิพนธ์ที่เกี่ยวข้อง โดยใช้เกณฑ์ PICO Framework ในการกำหนดขอบเขตของการศึกษา และใช้แนวทาง PRISMA Guidelines เพื่อควบคุมกระบวนการคัดเลือกงานวิจัย ผลการศึกษาพบว่า Multidisciplinary Transitional Care และ Home-based Care เป็นโปรแกรมที่มีประสิทธิภาพสูงสุด เพิ่มระดับความพร้อมก่อนออกจากโรงพยาบาล (RHDS Score) ได้เฉลี่ย 8.5 - 9.2 คะแนน (p < .001) และช่วยลดอัตราการกลับเข้ารักษาซ้ำได้ 25% - 35% (RR = 0.65 - 0.75, p < .001) นอกจากนี้ โปรแกรมดังกล่าวยังช่วยเพิ่มคุณภาพชีวิตของผู้ป่วยโดยเฉลี่ย 10% - 18% และได้รับคะแนนความพึงพอใจจากผู้ป่วยและครอบครัวสูงกว่าโปรแกรมที่ไม่มีการดูแลต่อเนื่องอย่างมีนัยสำคัญ (p < .05)
