ผลของโปรแกรมให้คำปรึกษาก่อนการบำบัดทดแทนไตต่อความรู้ และการตัดสินใจยอมรับการบำบัดทดแทนไต ของผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้าย

การวิจัยเป็นการวิจัยกึ่งทดลอง เพื่อเปรียบเทียบผลของโปรแกรมให้คำปรึกษาก่อนการบำบัดทดแทนไตต่อความรู้ และการตัดสินใจยอมรับการบำบัดทดแทนไตของผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้าย ที่เข้ามารับคำปรึกษาที่หน่วยไตเทียม 1 ในโรงพยาบาลสงขลา จำนวน 40 ราย คัดเลือกตามเกณฑ์คัดเข้าแบ่งเป็นกลุ่มควบคุมและกลุ่มทดลอง กลุ่มละ 20 ราย กลุ่มทดลองได้รับโปรแกรมให้คำปรึกษาก่อนการบำบัดทดแทนไต กลุ่มควบคุมได้รับการพยาบาลตามปกติ ประเมินความรู้และการตัดสินใจยอมรับการบำบัดทดแทนไตระหว่างกลุ่มทดลองและกลุ่มควบคุมก่อนและหลังการได้รับโปรแกรมฯ วิเคราะห์ข้อมูลด้วยสถิติพรรณนา Pair t-test และ Independent t-test ผลการศึกษาพบว่า ค่าเฉลี่ยคะแนนความรู้ของกลุ่มทดลองหลังได้รับโปรแกรมฯ ( =15.05, SD =1.43) สูงกว่าก่อนได้รับโปรแกรมฯ (
=10.00, SD = 2.38) และสูงกว่ากลุ่มควบคุม (
=13.80, SD =1.51) อย่างมีนัยสำคัญทางสถิติ (p < .05) และร้อยละการตัดสินใจยอมรับการบำบัดทดแทนไตของกลุ่มทดลองหลังได้รับโปรแกรมฯ สูงกว่า (ร้อยละ 50) ก่อนได้รับโปรแกรมฯ (ร้อยละ 40) และสูงกว่ากลุ่มควบคุม (ร้อยละ 35) ผลการศึกษาครั้งนี้แสดงให้เห็นว่า โปรแกรมฯ ช่วยเพิ่มความรู้และการตัดสินใจยอมรับการบำบัดทดแทนไต ดังนั้นพยาบาลวิชาชีพประจำหน่วยไตและบุคลากรทางการแพทย์ จึงต้องมีแนวปฏิบัติในการให้คำปรึกษาแก่ผู้ป่วยทุกรายก่อนการบำบัดทดแทนไต
