การพัฒนาระบบบริการเชิงรุกคัดกรองเอชไอวี ซิฟิลิสและไวรัสตับอักเสบซี ในผู้ต้องขังภายใต้โครงการราชทัณฑ์ปันสุข
บทคัดย่อ
ผู้ต้องขังคือพลเมืองไทยเมื่อเข้าสู่เรือนจำต้องได้รับการดูแลสุขภาวะเฉกเช่นคนปกติทั่วไป สิทธิ์ในการได้รับการดูแลด้าน สุขภาพ รักษาพยาบาลเป็นสิทธิขั้นพื้นฐานที่ประชาชนทุกคนควรได้รับ ถึงแม้ว่าจะต้องโทษจำคุก การเจ็บป่วยด้วยโรคติดต่อและ ไม่ติดต่อของผู้ต้องขัง อาการจะรุนแรงมากกว่าคนปกติทั่วไป เนื่องจากสภาพความเป็นอยู่ที่แออัดส่งผลทำให้โรคติดต่อมีการแพร่ กระจาย และวิถีชีวิตความเป็นอยู่ที่ไม่สะดวกสบาย และที่สำคัญเมื่อเจ็บป่วยเข้าถึงการรักษาได้ยาก การคัดกรองโรคก่อนเข้าสู่เรือนจำเป็นสิ่งจำเป็น เพื่อให้ทราบสถานะว่ามีความเจ็บป่วยอะไรบ้าง มีโรคติดต่อที่เป็นอันตรายร้ายแรงต่อผู้ต้องขังอื่นหรือไม่ เพราะ ผู้ต้องขังจะเป็นผู้นำพาเชื้อโรคมาสู่เรือนจำ
บทความนี้จะนำเสนอแนวทางขั้นตอนการคัดกรองผู้ต้องขัง อุปกรณ์ที่ต้องจัดเตรียม การเตรียมผู้ต้องขังก่อนการคัดกรอง บทบาทพยาบาลในการคัดกรองผู้ต้องขัง การเตรียมการของโรงพยาบาลแม่ข่าย และประโยชน์ที่จะได้รับจากการคัดกรองผู้ต้องขัง ซึ่งมีข้อมูลที่แสดงว่าระยะเวลาที่ควรคัดกรองผู้ต้องขังคือ แรกรับ เพื่อป้องกันการแพร่กระจายของโรคไปสู่ผู้ต้องขังรายอื่น และการคัดกรองก่อนปล่อยซึ่งจะเป็นการป้องกันไม่ให้โรคติดต่อแพร่กระจายจากเรือนจำสู่ชุมชน
