รูปแบบบริการสำหรับผู้สูงอายุที่มีภาวะพึ่งพาในสถานดูแลระยะยาว ในบริบทประเทศไทย

การวิจัยเชิงคุณภาพมีวัตถุประสงค์เพื่อศึกษารูปแบบบริการสำหรับผู้สูงอายุที่มีภาวะพึ่งพาในสถานดูแลระยะยาวใน ประเทศไทย ผู้ให้ข้อมูลเป็นผู้มีประสบการณ์ในการจัดบริการการดูแลผู้สูงอายุที่มีภาวะพึ่งพา จำนวน 48 คน ในภาคเหนือ ภาค ตะวันออกเฉียงเหนือ ภาคใต้ กรุงเทพมหานครและปริมณฑล เก็บรวบรวมข้อมูลโดยการสนทนากลุ่ม (Focus group discussion) การสังเกตการจัดบริการในสถานบริการภาครัฐและเอกชนที่ยินดีเข้าร่วมการวิจัย ได้แก่ โรงพยาบาล สถานบริบาล สถานสงเคราะห์ และบ้านพักมูลนิธิ ร่วมกับสังเคราะห์ข้อมูลทุติยภูมิได้แก่ รายงานประจำปี ผลประกอบการ แผนการจัดกิจกรรมและเอกสารต่าง ๆ ที่เกี่ยวข้องที่ได้รับอนุญาตให้ข้อมูล วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา (Content analysis)
ผลการศึกษาพบว่า 1. รูปแบบการจัดบริการระยะยาวในสถานดูแลระยะยาว (Long term care institution) ในประเทศไทย แบ่งเป็น 3 รูปแบบ ได้แก่ 1) สถานสงเคราะห์คนชรา/บ้านพักของมูลนิธิเอกชน 2) สถานบริบาลผู้สูงอายุภาคเอกชน และ 3) โรงพยาบาลรัฐบาลและเอกชน รูปแบบบริการทั้ง 3 มีลักษณะแตกต่างกันในประเด็นการดูแลด้านสุขภาพ กล่าวคือ ในสถาน สงเคราะห์คนชราหรือบ้านพักของมูลนิธิเอกชน เน้นการดูแลเชิงสังคมเป็นส่วนใหญ่ เริ่มต้นด้วยการสงเคราะห์ผู้สูงอายุที่ไม่มีที่อยู่ อาศัยและไม่มีผู้ดูแล ต่อมาปรับเป็นการดูแลสุขภาพตามความต้องการ ในสถานบริบาลผู้สูงอายุภาคเอกชน เริ่มต้นด้วยการดูแลผู้ สูงอายุที่ช่วยเหลือตนเองได้น้อยป่วยเรื้อรัง มีข้อจำกัดในการดูแลและครอบครัวไม่สามารถให้การดูแลได้ ส่วนในโรงพยาบาลเน้น การดูแลสุขภาพสำหรับผู้สูงอายุที่มีที่มีภาวะพึ่งพาโดยเฉพาะ 2. ลักษณะการจัดบริการดูแลระยะยาว ประกอบด้วย โครงสร้าง กายภาพและสิ่งแวดล้อม การประเมินจัดประเภทผู้สูงอายุ การดูแลสุขภาพร่างกาย การดูแลด้านสังคมและสังคมสงเคราะห์ ซึ่งแต่ละรูปแบบได้ดำเนินการตามความรู้และประสบการณ์ของผู้บริหารและผู้ปฏิบัติในระยะแรกและจากการศึกษาดูงานในบริบท อื่น ๆ ทั้งในประเทศและต่างประเทศ ต่อมาภายหลังได้ปรับปรุงตามมาตรฐานที่หน่วยงานกำหนดขึ้น ได้แก่ รูปแบบการดูแลใน สถานสงเคราะห์คนชรา/บ้านพักของมูลนิธิเอกชนและโรงพยาบาล ส่วนในสถานบริบาลเองไม่มีกฎหมายข้อกำหนดด้านมาตรฐาน การดูแลระยะยาวจากหน่วยงานภาครัฐ แต่ละแห่งจึงกำหนดขึ้นมาเองโดยเทียบเคียงกับข้อกำหนดมาตรฐานของสถานพยาบาลเนื่องจากยังมิได้มีการจัดทำมาตรฐานกลางหรือแนวปฏิบัติที่ชัดเจนสำหรับการดูแลระยะยาว ข้อเสนอแนะ ควรมีการพัฒนาปรับปรุง ระบบการดูแลระยะยาวและกำหนดกลไกการกำกับมาตรฐานให้เหมาะสมตามมาตรฐานการดูแลระยะยาวทั้งด้านมาตรฐานเชิง โครงสร้าง มาตรฐานการดูแล และมาตรฐานผลลัพท์ เพื่อให้สอดคล้องกับความต้องการการดูแลของผู้สูงอายุที่มีภาวะพึ่งพาในบริบท สังคมไทย
