การพัฒนาชุดตรวจ Osmotic Fragility Test สำหรับภาวะโลหิตจาง กรณีศึกษาโรงพยาบาลส่งเสริมสุขภาพตำบล จังหวัดเพชรบูรณ์

Abstract

งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาและพัฒนาการตรวจภาวะโลหิตจางด้วยเทคนิค Osmotic Fragility Test (OF Test) ในรพ.สต. (รพ.สต.) และพัฒนาความรู้ของบุคลากรในรพ.สต.เรื่องการตรวจคัดกรองภาวะโลหิตจางโดยจัดทำชุดตรวจ OF Test Kit และเก็บตัวอย่างเลือดจากข้อพับแขนของอาสาสมัคร จำนวน 115 ตัวอย่าง ตัวอย่างละ 1 มิลลิลิตร เพื่อทำการทดสอบความไวและความจำเพาะของชุดตรวจ รวมถึงการจัดอบรมเชิงปฏิบัติการการพัฒนาและเฝ้าระวังภาวะโลหิตจางและธาลัสซีเมียใน รพ.สต.จังหวัดเพชรบูรณ์ ให้แก่เจ้าหน้าที่สาธารณสุขซึ่งปฏิบัติงาน ใน รพ.สต. จังหวัดเพชรบูรณ์ เนื่องจากการตรวจคัดกรองภาวะโลหิตจาง ใน รพ.สต.จังหวัดเพชรบูรณ์มีข้อจำกัดในด้านเครื่องมือและความรู้ของบุคลากรด้านการตรวจคัดกรองภาวะโลหิตจาง ผลการวิจัยพบว่าจากการตรวจภาวะโลหิตจางด้วย OF Test Kit ซึ่งมีค่าความไวและความจำเพาะที่ร้อยละ 100 พบผู้ที่มีภาวะโลหิตจาง 39.13% และผู้ที่ไม่มีภาวะโลหิตจาง 60.87% และจากการจัดอบรมเชิงปฏิบัติการแก่เจ้าหน้าที่สาธารณสุขซึ่งปฏิบัติงานใน รพ.สต. จังหวัดเพชรบูรณ์ทั้งสิ้น 92 คน พบว่าค่าเฉลี่ยความรู้หลังการอบรมมีค่าสูงกว่าก่อนการอบรม อย่างมีนัยสำคัญทางสถิติ <0.01 สรุปได้ว่าชุดตรวจ OF Test Kit ที่พัฒนาขึ้นมีความง่ายรวดเร็ว และประหยัดค่าใช้จ่าย เนื่องจากอุปกรณ์ที่ใช้มีเพียงน้ำยาตรวจ (0.36% NaCl), Pasteur pipette และหลอดทดลอง และใช้ระยะเวลาในการทดลองเพียง 5 – 10 นาทีสามารถทำได้ภายใน 24 ชั่วโมง และบุคลากรมีความรู้ความเข้าใจภาวะโลหิตจาง มากขึ้น สามารถทำการตรวจคัดกรองด้วยเทคนิค OF Test เองได้ ทำให้ทราบอัตราความชุกของภาวะโลหิตจางในพื้นที่และสามารถนำชุดตรวจนี้ไปใช้ในการพัฒนารูปแบบการเฝ้าระวังภาวะโลหิตจางในจังหวัดเพชรบูรณ์ต่อไป

The Development of Osmotic Fragility Test Kit for Anemia:A Case Study in Tambol Health Promoting Hospital, Phetchabun Province, Thailand

The purposes of this study were to develop the test for anemia with osmotic fragility test (OF Test) for Tambol Health Promoting Hospital (THPH) in Phetchabun province and development of knowledge of personnel in THPH. Topic is screening for anemia. By providing OF Test Kit and collected blood samples from people in the area of 115 samples per 1 milliliter for sensitivity & specificity of testing. Include, conducted for anemia and thalassemia surveillance to personnelin THPH in Phetchabun province.Because of the screening of anemia for THPH still lack of knowledge, technique and equipments. The result showed that screening for anemia with OF Test Kit, which has a sensitivity and specificity of 100% found people 39.13% were anemia and 60.78% were normal. And pre-test and post-test were conduct for their knowledge of to health personnel 92 person in THPH in Phetchabun province. The result confirm, the increasing of their knowledge with statistical significance (p<0.01). In conclusion, The training for OF Test Kit is easy, fast and cost. Because of the device use only reagents (0.36% NaCl), Pasteur pipette and test tube, and take a period of trial only 5 – 10 minutes can be done within 24 hour, and found that all participants understand and can follow the test kit procedure and we confirmed the OF Test kit and our training can increase their knowledge and can read them to do screening test for anemia and thalassemia in their community for surveillance.
