Effectiveness of Self-Regulation Enhancement Program in Preventing Central Line-Associated Bloodstream Infection among Nurses in Intensive Care Units

Background: Central line-associated bloodstream infection (CLABSI) increases healthcare costs and patient mortality. Nurses play a crucial role in preventing this infection, particularly in intensive care units.
Objective: To examine the effectiveness of a self-regulation enhancement program in preventing CLABSI among nurses in intensive care units.
Methods: A quasi-experimental, two-group pretest–posttest design was conducted among 48 intensive care unit nurses, who were divided into an experimental group and a control group, with 24 participants in each group. The experimental group received the self-regulation promotion program for 14 weeks. The research instruments included a personal information questionnaire, an observational checklist of infection prevention practices, and a CLABSI incidence record form. The content validity index was 0.98, and the reliability of the observational checklist was 1.00. Data were analyzed using descriptive statistics, Chi-square test, and Fisher’s exact test.
Results: After receiving the program, the proportion of correct CLABSI prevention practices in the experimental group increased from 64.31% to 99.26% (p < .001) and was significantly higher than that of the control group, which had a correct practice rate of 72.14% (p < .001). The CLABSI incidence rate in the experimental group decreased from 7.98 to 2.92 episodes per 1,000 central line-days (p = .348) and was 1.95 times lower than that of the control group (p = .548).
Conclusion: The self-regulation promotion program was associated with improved correct practices for CLABSI prevention among intensive care unit nurses and showed a tendency to reduce CLABSI incidence.
