Effectiveness of Continuity of Care Program on Hospital Discharge Readiness in Surgical and Medical Patients in Asia : A Systematic review

This study is a systematic review and meta-analysis aimed at evaluating the effectiveness of transitional care programs on readiness for hospital discharge (RHDS) and hospital readmission rates among surgical and medical patients in Asia. Relevant studies were selected from internationally recognized academic databases, including PubMed, Scopus, Web of Science, CINAHL, Cochrane Library, Embase, and ScienceDirect, using PICO Framework to define the study scope and PRISMA Guidelines to ensure a standardized selection process. The findings indicate that Multidisciplinary Transitional Care and Home-based Care were the most effective programs, significantly improving RHDS Score by 8.5 - 9.2 points (p < .001) and reducing hospital readmission rates by 25% - 35% (RR = 0.65 - 0.75, p < .001). Moreover, these programs enhanced patient quality of life (QoL) by 10% - 18% and achieved significantly higher patient and family satisfaction scores (p < .05) compared to standard care without transitional support.
