The Development of Nursing Services for Patients with Coronary Artery Bypass Graft at Sakon Nakhon Hospital

Abstract

This research and development aimed to develop and evaluate a nursing service for the patients with coronary artery bypass graft(CABG) at Sakon Nakhon Hospital. The Mahdjoubi (2009) research methodology was 6 steps and conducted during October 2019-September 2021. The purposive samples were 10 chief nursing officers, 36 registered nurses and 66 patients with CABG. The research tools were composed of the developed a nursing service for patients with CABG (CVI 0.8) consisted of the developed a case management nursing service, clinical nursing practice guideline and nursing specific competency. Data collection instruments consisted of a nursing competency assessment form(Cronbach’s Alpha Coefficient 0.7),focus group questionnaire(CVI 0.8) and satisfaction assessment of nursing service(CVI 0.8). Data were analyzed using group content, mean, percentile, Chi-square test and Wilcoxon Signed Ranks test.

The results revealed that: 1) The nursing service system for patients with CABG consisted of 3 components; (1) nursing service system using a case management model. (2) Clinical nursing practice guidelines based on King’s theory including the nursing standard for surgical patients and critical care for 3 parts ; pre-intra and post operative nursing and (3) the 8 specific competencies and 2) The results of the developed nursing service system found that (1)the incident of unpreparedness postponing surgery, patient’s worsens unexpected, surgical wound infection, unexpected death and the specific competency showed significantly, readmission in 28 days showed non significantly (p<.05), and the health care provider were highest satisfaction.
