การศึกษาปัจจัยส่งเสริมการตัดสินใจเชิงจริยธรรมที่เกี่ยวกับควำมเสี่ยงทำงการพยาบาลในคลินิกอย่างมีประสิทธิผลของพยาบาลทหารบก

Abstract

การวิจัยครั้งนี้ เป็นการวิจัยเชิงคุณภาพ ศึกษาปัจจัยส่งเสริมการตัดสินใจเชิงจริยธรรมที่เกี่ยวกับความเสี่ยงทางการพยาบาลในขณะปฏิบัติการพยาบาลอย่างมีประสิทธิผลของพยาบาลทหารบก กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพในโรงพยาบาลทหารบก 2 แห่ง จำนวน 62 คน โดยใช้เทคนิค focus group จำนวน 5 กลุ่ม รวมทั้งการสังเกตพร้อมทั้งศึกษาเอกสารที่เกี่ยวข้องและการสัมภาษณ์เชิงลึกจากผู้เชี่ยวชาญจำนวน 5 ท่านเพื่อยืนยันผลการวิจัย เก็บรวบรวมข้อมูลแล้วทำการวิเคราะห์โดยใช้เทคนิค Content analysis
ผลการวิจัย พบว่า ปัญหาจริยธรรมที่พบในโรงพยาบาลทหารบก มีประเด็นสำคัญ คือ 1) พฤติกรรมบริการ การให้ข้อมูลทำให้ผู้ป่วยไม่พึงพอใจ 2) การประเมินอาการและให้การช่วยเหลือล่าช้า ทำให้ผู้ป่วยไม่ปลอดภัยและไม่ได้รับการดูแลช่วยเหลือที่เหมาะสม เนื่องจากภาระงานมากและทีมงานไม่มีความสามารถช่วยเหลือได้ 3) การดูแลผู้ป่วยระยะสุดท้ายในการจัดบริการที่ต้องทำความเข้าใจระหว่างทีมงานและผู้ป่วยรวมทั้งญาติผู้ดูแลที่หลากหลายระบบ การดำเนินงานที่ยังไม่ชัดเจนใน 4) ความผิดพลาดจากการปฏิบัติการพยาบาล ปัจจัยสำคัญที่ส่งเสริมการตัดสินใจเชิงจริยธรรมประกอบด้วย ปัจจัยภายนอก ได้แก่ การพัฒนาความรู้ด้านจริยธรรมและการพยาบาล การจัดการอุปกรณ์ เครื่องมือเครื่องใช้ ภาระงานที่เหมาะสม การนิเทศ วัฒนธรรมชุมชน สถานที่จัดบริการ ปัจจัยภายใน ได้แก่ คุณค่าในตนที่เป็นประโยชน์ต่อผู้ป่วย สังคมและการได้รับการยอมรับ สภาพจิตใจอารมณ์ ส่วนปัจจัยเกี่ยวกับองค์กรและการสนับสนุนทางสังคม ได้แก่ นโยบายและทิศทางการจัดการทรัพยากร การให้ข้อมูลและข้อมูลย้อนกลับ เป็นต้น ผลการศึกษาครั้งนี้แสดงให้เห็นว่าปัจจัยส่งเสริมการตัดสินใจเชิงจริยธรรมที่เกี่ยวกับความเสี่ยงทางการพยาบาล มีหลายปัจจัย ดังนั้น ผู้บริหารทางการพยาบาลควรนำผลการศึกษานี้ไปใช้ในการวางแผนการพัฒนาเพื่อส่งเสริมการตัดสินใจเชิงจริยธรรมเกี่ยวกับความเสี่ยง เพื่อช่วยให้พยาบาลตัดสินใจอย่างมีประสิทธิผลและลดปัญหาในการปฏิบัติการพยาบาลต่อไป

The Study of the Factors Related with Ethical Decision-Making on Nursing Risk in Clinical Practice of Army Nursing Staff

This qualitative research was to the study of the promoting ethical decision making factors which related with the nursing practice risk among RTA nurses. 62 sample size of professional nurses from 2 Army hospitals were collected. The data collected by using focus group discussion techniques, in-depth interview, observation and reviewed related documents. The content analysis was analyzed and confrmed by the in-depth interview of 5 expertizes. The result found that the important issues of ethical practice problems were 1) Service behavior such as client unsatisfed with the nurse in providing information 2) The delay of assessment and services made client unsafe and in-appropriated service due to the work load. 3) On the issue of palliative care service needs the understanding among team works all include the patient, relative and caregiver which is still not clear in
this system 4) the last is the errors in nursing practice Key factors that promote ethical decision making should include of external factors such as ethical and nursing knowledge development, providing medical equipment, appropriated workloads, nursing supervision and service location. For the internal factors including intrinsic value which beneft to the patient, the acceptance from society and the emotional stage. About the factor of
organization and social support were found of the policy and the resource management setting, providing and giving feedback information. According to the research fndings, the factors related with Ethical Decision-making on Nursing Risk have many factors. The results of this study can be used as a guideline for developmental for Ethical Decisionmaking on Nursing Risk and reduce problems in nursing practice.
