Breastfeeding Situation, Facilitators and Obstacles, Policy and Program to Promote Breastfeeding in Thailand

Abstract

Breastfeeding has been a traditional infant feeding practice in Thailand. World Health Organization (WHO) recommends exclusive breastfeeding (EBF) during the first 6 months and complementary feeding thereafter, with continued breastfeeding till two years of age. Rapid national development results in changing people’s living environment and lifestyles, including infant feeding practices. This literature review aims to examine the trends in breastfeeding, factors influencing (facilitators and barriers) breastfeeding, policy and programs that support and promote breastfeeding in Thailand. Findings: Prevalence of EBF for 6 months has increased, but remains low (23%); breastfeeding till one and two years were 33 and 15.6%, in 2016.  Culture/tradition and beliefs, maternal knowledge, attitudes, self efficacy and working outside home, family support, socio-economic status are important facilitators or barriers to breastfeeding. National policy and program are mainly devised by the Ministry of Public Health, working in collaboration with other government agencies, academic institutions, and civil society, in providing environment conducive to breastfeeding.  Key policies and programs include maternity leave (3 months), mandatory code of marketing of breast milk substitute (‘Milk Code Act’), provision of breastfeeding supports in health service facilities, capacity building of health personnel and lactation management, breastfeeding corner in work place, and public awareness campaigns. Further research is needed to strengthen program implementation through improved knowledge management of new knowledge in the Thai context. Strengthening program effectiveness by the use of new technology suitable with the current lifestyles in promotion, support and protect breastfeeding. Finally, model and system for monitoring and evaluation of policy implementation and program effectiveness on prevalence and quality of breastfeeding in Thailand should be established.
