Development of an Islamic-Based Care Model to Reduce Pain and Fear Among Laboring Women

Abstract
Background: Pain and fear during the waiting phase of labor can adversely affect childbirth outcomes, particularly among Muslim primigravidas.
Objectives: This study aimed to: 1) explore problems and registered nurses’ needs concerning the care of women during the waiting phase of labor; 2) develop a care model based on Islamic principles to reduce pain and fear; and 3) evaluate the effectiveness of the developed model.
Methods: This research and development study comprised three phases. Phase 1 involved a qualitative study of problems and care needs among 15 registered nurses. Phase 2 focused on developing a care model for women during the waiting phase of labor. Phase 3 evaluated the model’s effectiveness using a quasi-experimental design involving 52 Muslim primigravidas, who were assigned to experimental and control groups. Data were analyzed using descriptive statistics, paired-samples t-tests, and independent-samples t-tests.
Results: The findings revealed that: 1) nurses needed to constantly explain pain and fear management to women awaiting childbirth and their relatives, indicating a need for innovative educational materials integrating spiritual dimensions; 2) the developed care model, called the “YARING Model,” comprised six components: faith (Yakin), practice (Amal), love (Rasa Cinta), knowledge (Ilmu), gratitude (Nikmat), and inner movement (Gerak Batin); and 3) after the intervention, the experimental group had significantly lower mean pain and fear scores than the control group (p<.05).
Conclusion: The YARING Model effectively reduced pain and fear among women during the waiting phase of labor. Hospitals should implement this model to promote care that aligns with the spiritual values of Muslim women.
