Development of a Nursing Service System for Pregnant Women with Anemia at the Antenatal Care Department, Trang Hospital

Abstract

Background: The prevalence of anemia among pregnant women both nationwide and in Trang Province has continued to rise. This remains an important health problem that directly affects the health of both mothers and their fetuses.


Objectives: To develop and evaluate the effectiveness of a healthcare service system for pregnant women with anemia.


Methods: This research and development study was conducted from May 2024 to July 2025 and comprised four phases: (1) situation analysis through a literature review and in-depth interviews with 10 pregnant women with anemia and three registered nurses; (2) development of a healthcare service system; (3) implementation of the developed system among 30 pregnant women with anemia using a one-group pretest–posttest quasi-experimental design; and (4) evaluation of the effectiveness of the healthcare service system. Data were analyzed using descriptive statistics and a dependent t-test.


Results: The developed healthcare service system consisted of three main components: (1) structure, involving the establishment of an anemia clinic; (2) process, involving health education and promotion of self-care; and (3) outcomes, involving monitoring of hematocrit levels. Evaluation of the system showed that, after the intervention, participants had improved knowledge and self-care behaviors. In addition, the participants’ mean hematocrit level at the time of delivery was significantly higher than that before the intervention (p<.001).


Conclusion: The developed healthcare service system significantly reduced the incidence of anemia and improved self-care behaviors among pregnant women. Therefore, this system represents an effective and appropriate approach for improving the quality of care in antenatal care units.
