The Development of a Model for Preventing Phlebitis in Newborns at a Hospital in Songkhla Province

Abstract

Background: Neonatal phlebitis is an inflammation of the vein at the site of intravenous infusion. It is a common complication and may lead to adverse outcomes, affecting neonatal safety and the quality of nursing care.


Objectives: To study the current situation, develop, and evaluate the effectiveness of a preventive model for phlebitis in neonates.


Methods: This research and development study was conducted in three phases: (1) document analysis and qualitative data collection involving 54 registered nurses; (2) development of a phlebitis prevention model with a sample of 10 registered nurses; and (3) evaluation of the model’s effectiveness using a quasi-experimental design with 54 registered nurses and newborns. Research instruments included a compliance assessment form for the prevention model and an intravenous infusion assessment form. Data were analyzed using descriptive statistics and a one-sample t-test.


Results: The situational analysis revealed that major causes of neonatal phlebitis included administration of antibiotics or high-concentration solutions, uncontrolled neonatal movement, and insufficient nursing knowledge regarding medication administration. The developed model comprised clinical practice guidelines for phlebitis prevention, an intravenous infusion assessment tool, and a limb stabilization device for infusion sites. Following implementation of the model, the incidence of phlebitis decreased to 5.60%. and nurses demonstrated a high level of compliance with the model (M=99.35, SD=1.69).


Conclusion: The developed preventive model for neonatal phlebitis effectively reduced the incidence of phlebitis and is appropriate for application in neonatal nursing practice.
