Risk Factors and Outcomes of Severe Pneumonia in Pediatric Patients with Influenza: An Analysis of HosXP Data from a Tertiary Hospital in Thailand

Abstract

Background: Influenza-associated pneumonia remains a major cause of morbidity among children worldwide. However, data on prognostic factors for severe influenza-related pneumonia in Thailand are limited.


Objectives: To identify independent risk factors and clinical outcomes of severe pneumonia in hospitalized pediatric patients with influenza-associated pneumonia.


Methods: This retrospective cohort study analyzed electronic medical records from the HosXP system at Chaiyaphum Hospital between 2019 and 2025. A total of 244 pediatric patients aged 0–15 years diagnosed with influenza and pneumonia were included. Pneumonia severity was categorized into three levels as mild, moderate, and severe. Prognostic factors were analyzed using ordinal logistic regression.


Results: The study revealed that 12.7% of patients required ventilatory support. Independent risk factors for severe pneumonia included underlying comorbidities (adjusted OR 5.97; 95% CI 2.31–15.43), tachypnea (adjusted OR 5.55; 95% CI 2.01–15.33), increased heart rate (adjusted OR 1.05 per 1 beat/min; 95% CI 1.03–1.08), and elevated neutrophil percentage (adjusted OR 1.04 per 1% increase; 95% CI 1.01–1.07). Hospital length of stay and ICU admission rates increased with disease severity.


Conclusion: Early assessment of comorbidities, respiratory distress, heart rate, and neutrophil percentage at admission can help identify children at risk for severe pneumonia and support clinical decision-making to effectively reduce adverse outcomes.
