Effects of the discharge planning program for patients with heart failure on self-care behavior and hospital readmissions

Abstract

Background: Heart failure is a significant health issue that requires continuous care. Discharge planning plays a crucial role in helping patients maintain proper self-care and reducing hospital readmissions.


Objective: To examine the effect of the discharge planning program for patients with heart failure on self-care behaviors and hospital readmissions. 


Methods: This quasi-experimental research was conducted using a one-group pretest-posttest design. The sample consisted of 31 participants who were diagnosed with heart failure by physicians and admitted to the hospital between December 2023 and March 2024. Participants were selected using purposive sampling. The intervention instrument was a discharge planning program for patients with heart failure. Data collection tools included a self-care behavior questionnaire and a hospital readmission record form. The data were analyzed using descriptive statistics and paired t-tests.


Results: The results showed that the mean score of self-care behaviors was significantly higher after the intervention than before the intervention (p<.001), and no patient was readmitted within 28 days.


Conclusions: Nurses and healthcare professionals can apply this program to discharge planning to enhance the quality of care for patients with heart failure, ensuring continuous care between community hospitals and primary care units.
