Factors influencing hypoglycemia in patients with type 2 diabetes mellitus receiving care at a diabetes outpatient clinic

Abstract
Background: Hypoglycemia is an acute and life-threatening complication and a significant factor hindering the management of glycemic control in patients with diabetic mellitus (DM).
Objective: The aim of this study was to identify factors affecting hypoglycemia in patients with type 2 DM treated in a diabetes outpatient clinic.
Methods: A retrospective descriptive study was conducted with 3-year medical records of patients with type 2 DM, in which 100 patients with type 2 DM with hypoglycemia were included as a case group. The researchers set a case-to-control ratio of 1 : 2. Thus, the control group consisted of 200 patients with type 2 DM without hypoglycemia . The research instrument was a patient record form of patient characteristics, disease data, and treatment data. Data were analyzed using t-test, chi-square test, and multiple logistic regression.
Results: The study found that duration of illness (> 5 years, OR 2.28, [95% CI 1.12 - 4.63]), body mass index (< 18.5 kg/m2, OR 0.18, [95% CI 0.07 - 0.45]), eGFR (< 60 ml/min/1.73m2, OR 4.04, [95% CI 2.08 - 7.87]), number of oral antihyperglycemic medication (> 1 medication, OR 2.47, [95%CI 1.31 - 4.67]), and antihypertensive medication use (received, OR 2.75, [95%CI 1.44 - 5.23]) were all factors associated with the occurrence of hypoglycemia (p<.05).
Conclusions: Factors influencing the occurrence of hypoglycemia in type 2 DM include duration of illness, body mass index, eGFR, number of oral antihyperglycemic medications, and the use of antihypertensive medication.
