The development model of case management for disable seniors in community: A case study of urban communities in Nonthaburi municipality

Abstract
The purpose of this article is to present a developed case management model for disable seniors in the community. The model was developed based on community and network partner collaboration to explore the problems and assess the community's potential for building health care systems in congruence with the needs of seniors and context of the community. Case management standards established by Case Management Society of America, including 1) Client identification and selection; 2) Assessment and problem/opportunity identification; 3) Development of the case management plans; 4) Implementation and coordination of care activity; 5) Evaluation of the case management plans and follow-up; and 6) Termination of the case management were used for model development. The case management model is divided into two approaches, namely, partial dependent care (for seniors ADL 5-11) and total dependent care (for seniors ADL< 5). Based on the lesson learnt from 3 communities Nonthaburi municipality and their partnerships, they satisfied with how the model could be used to help disable senior people in community and how the model could enhance the effectiveness of referral system. Consequently, this model will advocate families, community and partner organizations working together effectively for disable seniors in the community. In addition, the elderly care system can be delivered seamlessly through this model to enhance elderly quality of life in Nonthaburi municipality and other communities.
