ผลลัพธ์ทางคลินิกของการรักษาโรคมะเร็งในผู้สูงอายุที่มีโรคร่วม : การทบทวนวรรณกรรมอย่างเป็นระบบ

บทคัดย่อ
ในปัจจุบันผู้ป่วยโรคมะเร็งที่มีโรคร่วมมีจำนวนสูงขึ้น เนื่องจากจำนวนประชากรผู้สูงอายุที่เพิ่มขึ้น ซึ่งโรคร่วมอาจส่งผลต่อการวินิจฉัย การรักษา และผลลัพธ์ทางคลินิกของผู้ป่วยมะเร็ง แม้จะมีฉันทามติเกี่ยวกับวิธีการบันทึก ตีความ หรือจัดการโรคร่วมในบริบทของผู้ป่วยโรคมะเร็ง แต่พบว่า ผลลัพธ์ทางคลินิกในผู้ป่วยที่เป็นโรคร่วมมีโอกาสน้อยที่จะได้รับการรักษาด้วยความตั้งใจที่จะรักษา วัตถุประสงค์ของการทบทวนวรรณกรรมอย่างเป็นระบบครั้งนี้ เพื่อรวบรวมงานวิจัยเกี่ยวกับผลลัพธ์ทางคลินิกของการรักษาโรคมะเร็งในผู้สูงอายุที่มีโรคร่วม และสังเคราะห์ผลลัพธ์ทางคลินิกของการรักษาโรคมะเร็งในผู้สูงอายุที่มีโรคร่วม ระหว่างปี พ.ศ. 2555-2564 โดยสืบค้นอย่างเป็นระบบจากฐานข้อมูลอิเล็กทรอนิกส์ ทั้งงานวิจัยในประเทศไทยและต่างประเทศ พบงานวิจัยที่ตรงตามเกณฑ์การคัดเข้าและการประเมินคุณภาพงานวิจัย จำนวน 10 เรื่อง ผลการศึกษา พบว่า อัตราการรอดชีวิต ระยะเวลาการปลอดจากโรคมะเร็ง และอัตราการกลับซ้ำของโรคมะเร็งที่เป็นปัจจัยการพยากรณ์ของโรคมะเร็งจะแย่ลงตามความรุนแรงของโรคร่วมและอายุที่เพิ่มขึ้น ดังนั้นจากผลการศึกษาชี้ให้เห็นถึงความสำคัญและความจำเป็นของการประเมินและทบทวนแนวทางการดูแลโรคร่วมในผู้สูงอายุโรคมะเร็ง ซึ่งเป็นกลยุทธ์ในการช่วยแก้ไขปัญหา รวมถึงการปรับปรุงฐานข้อมูลสำหรับผู้ป่วยที่มีโรคร่วม การพัฒนาเครื่องมือทางคลินิกที่มีอยู่ให้สอดคล้องกับผู้ป่วย จะช่วยในการตัดสินใจเกิดการบูรณาการการดูแลรักษาที่ดีขึ้น เพื่อปรับปรุงผลลัพธ์ทางคลินิกในผู้สูงอายุโรคมะเร็งที่มีโรคร่วมต่อไป
