บทคัดย่อ

การวิจัยเชิงปฏิบัติการแบบผสมผสานนี้ มีวัตถุประสงค์เพื่อพัฒนาและประเมินประสิทธิภาพของระบบเฝ้าระวังเชื้อดื้อยาต้านจุลชีพแบบดิจิทัล โรงพยาบาลวารินชำราบ จังหวัดอุบลราชธานี ดำเนินการระหว่างเดือนกรกฎาคม 2567 ถึงกันยายน 2568 โดยพัฒนาระบบผ่านเทคโนโลยีที่ทำให้ผู้ที่ไม่มีทักษะในการเขียนโปรแกรมสามารถสร้างแอปพลิเคชันได้ ประกอบด้วย Google Forms, Google Sheets, Looker Studio และ Line Notify API เพื่อแก้ไขข้อจำกัดของระบบเดิมที่ล่าช้าและใช้การบันทึกด้วยมือ ซึ่งมีระยะเวลาการแจ้งเตือนเชื้อดื้อยาเฉลี่ยถึง 6 ชั่วโมง กลุ่มตัวอย่างคือบุคลากรทางการแพทย์ผู้ใช้งานหลัก 50 คน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการสัมภาษณ์เชิงลึก ผลการศึกษาพบว่า ระบบที่พัฒนาขึ้นสามารถแจ้งเตือนเชื้อดื้อยาได้ในเวลาเพียง 5 นาที และให้ข้อมูลที่เป็นปัจจุบันแบบเรียลไทม์ ร้อยละ 100 จากรายงานการเฝ้าระวังทั้งหมด 915 รายงานในผู้ป่วย 566 ราย พบเชื้อดื้อยาสำคัญคือ Acinetobacter baumannii (CRAB) และ Klebsiella pneumoniae (CRKP) โดยพบความชุกสูงสุดในหอผู้ป่วยหนักอายุรกรรม ร้อยละ 19.5 (178/915 รายงาน) บุคลากรมีความพึงพอใจต่อระบบในระดับมาก (ค่าเฉลี่ย 4.28 ส่วนเบี่ยงเบนมาตรฐาน 0.79) และของผู้ใช้งานสามารถนำข้อมูลไปใช้ประกอบการตัดสินใจรักษาและควบคุมการระบาดได้ทันที ร้อยละ 84 โดยสรุปการพัฒนาระบบเฝ้าระวังเชื้อดื้อยาต้านจุลชีพด้วยเทคโนโลยีที่เข้าถึงง่ายและไม่มีค่าใช้จ่าย สามารถเพิ่มประสิทธิภาพของกระบวนการเฝ้าระวังและการสื่อสารข้อมูลในโรงพยาบาลได้อย่างมีนัยสำคัญ ช่วยสนับสนุนการตัดสินใจทางคลินิกและการควบคุมการติดเชื้อ
