บทนำ ความคลาดเคลื่อนจากการสั่งใช้ยา เป็นเหตุการณ์ที่อาจนำไปสู่การใช้ยาที่ไม่เหมาะสมหรืออันตรายแก่ผู้ป่วย นอกจากนี้ยังเป็นหนึ่งในตัวชี้วัดคุณภาพที่สำคัญของโรงพยาบาลอีกด้วย การวิเคราะห์อุบัติการณ์ที่เกิดขึ้นทำให้ทราบปริมาณความถี่และสาเหตุที่ควรแก้ไขเพื่อนำไปสู่ความปลอดภัยจากการใช้ยา
วัตถุประสงค์ เพื่อศึกษาชนิดและความถี่ของการเกิดความคลาดเคลื่อนจากการสั่งใช้ยา และวิเคราะห์หาสาเหตุของความคลาดเคลื่อนจากการสั่งใช้ยา
วิธีการศึกษา เป็นการวิจัยเชิงพรรณนา โดยการนำข้อมูลจากคำสั่งแพทย์ที่มีการบันทึกคำสั่งใช้ยาในระบบคอมพิวเตอร์ของผู้ป่วยในในเวลาราชการ ช่วงวันที่ 1 กันยายน 2566 – 29 กุมภาพันธ์ 2567 และเก็บข้อมูลในแบบรายงานความคลาดเคลื่อนทางยา นำมาวิเคราะห์ข้อมูลทางสถิติ
ผลการศึกษา ความคลาดเคลื่อนจากการสั่งใช้ยาทั้งหมด 549 ครั้ง ความคลาดเคลื่อนที่พบมากที่สุด 5 อันดับ คือ การสั่งขนาดยาสูงเกินไป จำนวน 186 ครั้ง (ร้อยละ 33.88) การสั่งขนาดยาต่ำเกินไป จำนวน 118 ครั้ง (ร้อยละ 21.49) การสั่งยาผิดรายการ จำนวน 114 ครั้ง (ร้อยละ 20.77) การไม่สั่งยาที่ผู้ป่วยควรจะได้รับ จำนวน 45 ครั้ง (ร้อยละ 8.20) และการสั่งยาซ้ำซ้อน จำนวน 22 ครั้ง (ร้อยละ 4.01)
สรุป จากการศึกษาความคลาดเคลื่อนจากการสั่งใช้ยาที่พบมากที่สุด คือ การสั่งขนาดยาสูงเกินไป ดังนั้นจึงควรหาแนวทางในการพัฒนาระบบให้มีการป้องกันการเกิดความคลาดเคลื่อนจากการสั่งใช้ยาและเก็บรวบรวมข้อมูลเพื่อติดตามประสิทธิภาพหลังจากการพัฒนาระบบอย่างต่อเนื่อง
