ปัจจัยคัดสรรที่สัมพันธ์กับภาระการดูแลของผู้ดูแลหลักในครอบครัวของผู้สูงอายุที่มีโรคเรื้อรังร่วมหลายโรค

บทคัดย่อ

การวิจัยเชิงพรรณนาแบบภาคตัดขวางนี้ มีวัตถุประสงค์เพื่อศึกษาภาระการดูแลและปัจจัยที่มีความสัมพันธ์กับภาระการดูแลของผู้ดูแลหลักในครอบครัวของผู้สูงอายุที่มีโรคเรื้อรังร่วมหลายโรค ตัวอย่าง คือ คู่ผู้ดูแลหลักและผู้สูงอายุ จำนวน 101 คู่ ที่มารับบริการ ณ แผนกผู้ป่วยนอกอายุรกรรม โรงพยาบาลรัฐระดับตติยภูมิ 3 แห่งในกรุงเทพมหานคร ได้มาโดยการสุ่มตามสะดวก เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคลและเครื่องมือมาตรฐานที่มีความตรงในการวัดและประเมิน ได้แก่ แบบประเมินกิจวัตรประจำวัน แบบประเมินภาวะเปราะบาง แบบประเมินความรู้สึกเป็นภาระในการดูแล แบบประเมินคุณภาพการนอนหลับ แบบประเมินการสนับสนุนทางสังคมและแบบประเมินภาวะซึมเศร้า ผลการทดสอบความเที่ยงได้ค่าสัมประสิทธิ์แอลฟาของครอนบาค เท่ากับ .90, .74, .92, .76, .91 และ .90 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงบรรยาย สถิติสหสัมพันธ์อีต้า สถิติสหสัมพันธ์แบบสเปียร์แมน และสถิติสหสัมพันธ์แบบเพียร์สัน

ผลการวิจัยพบว่า ผู้ดูแลมีความรู้สึกถึงภาระในการดูแลระดับไม่เป็นภาระถึงเป็นภาระเล็กน้อย (M = 21.24, SD = 13.82) ผลการวิเคราะห์ความสัมพันธ์ระหว่างปัจจัยคัดสรรที่สัมพันธ์กับภาระในการดูแล พบว่า ภาวะซึมเศร้าของผู้ดูแล (r = .414, p < .05) คุณภาพการนอนหลับที่ไม่ดีของผู้ดูแล (r = .371, p < .05) และความเปราะบางทางร่างกายของผู้สูงอายุ (r = .350, p < .05) มีความสัมพันธ์ทางบวกระดับปานกลางกับภาระในการดูแล อย่างมีนัยสำคัญทางสถิติ ในขณะที่ความสามารถในการทำกิจวัตรประจำวันของผู้สูงอายุมีความสัมพันธ์ทางลบระดับปานกลางกับภาระในการดูแลอย่างมีนัยสำคัญทางสถิติ (r = −.435, p < .05) และการสนับสนุนทางสังคมของผู้ดูแลมีความสัมพันธ์ทางลบระดับต่ำกับภาระในการดูแล อย่างมีนัยสำคัญทางสถิติ (r = −.210, p < .05) อย่างไรก็ตาม พบว่า เพศของผู้สูงอายุและระดับการศึกษาของผู้ดูแลไม่มีความสัมพันธ์กับภาระในการดูแล (η = .07 และ r_s = −.096, p > .05)

ดังนั้น การลดภาระในการดูแลของผู้ดูแลหลักในครอบครัวของผู้สูงอายุที่มีโรคเรื้อรังร่วมหลายโรค ควรให้ความสำคัญกับผู้ดูแลในการประเมินและการจัดการภาวะซึมเศร้า การส่งเสริมคุณภาพการนอนหลับ รวมทั้ง การเสริมสร้างการสนับสนุนทางสังคม และในผู้สูงอายุควรมุ่งเน้นการป้องกันและลดความเปราะบางทางร่างกาย รวมทั้ง การส่งเสริมความสามารถ
ในการทำกิจวัตรประจำวัน เพื่อนำไปใช้เป็นแนวทางในการวางแผน ออกแบบกิจกรรมการพยาบาล และพัฒนากลยุทธ์เพื่อลดภาระในการดูแลได้อย่างมีประสิทธิภาพต่อไป
