การพัฒนาระบบบริการผู้ป่วยติดเตียงที่ยากไร้ พื้นที่กรุงเทพมหานคร

งานวิจัยผสมวิธีนี้มีวัตถุประสงค์เพื่อ 1) ศึกษาระดับของการพัฒนาระบบบริการผู้ป่วยติดเตียงที่ยากไร้ นวัตกรรมบริการ มาตรฐานดูแลรักษาพยาบาล นโยบายระบบบริการผู้ป่วยติดเตียงที่ยากไร้ และภาคีการมีส่วนร่วม 2) เพื่อศึกษาอิทธิพลของนวัตกรรมบริการ มาตรฐานดูแลรักษาพยาบาล นโยบายระบบบริการผู้ป่วยติดเตียงที่ยากไร้ และภาคีการมีส่วนร่วม ที่มีผลต่อการพัฒนาระบบบริการผู้ป่วยติดเตียงที่ยากไร้ และ 3) เสนอแนะแนวทางการพัฒนาระบบบริการ ตัวอย่างในการวิจัยเชิงปริมาณเป็นผู้ป่วยติดเตียง จำนวน 350 คน ได้มาโดยสุ่มตัวอย่างแบบหลายขั้นตอน วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและการวิเคราะห์โมเดลสมการเชิงโครงสร้าง (SEM) และ Path Analysis สำหรับการวิจัยเชิงคุณภาพได้มาจากผู้ให้ข้อมูลในการสัมภาษณ์เชิงลึก จำนวน 15 ราย คัดเลือกแบบเฉพาะเจาะจง และวิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา
ผลการวิจัยพบว่า 1) ด้านที่มีค่าเฉลี่ยสูงสุด คือ ด้านการพัฒนาระบบบริการผู้ป่วยติดเตียงที่ยากไร้ รองลงมา คือ ด้านมาตรฐานดูแลรักษาพยาบาล นโยบายระบบบริการผู้ป่วยติดเตียงที่ยากไร้ นวัตกรรมบริการ และด้านภาคีการมีส่วนร่วม ตามลำดับ 2) ผลการวิเคราะห์ข้อมูลเชิงประจักษ์กับโมเดลสมการโครงสร้างที่ได้มาจากทบทวนวรรณกรรมพบว่า มีความเหมาะสมและสอดคล้องกลมกลืนกันดี (Chi-square = 70.9, df = 44, p = .06 , GFI = .99, AGFI =.99, CFI = .99, RMR = .043, RMSEA = .042) โดยมาตรฐานดูแลรักษาพยาบาลมีอิทธิพลโดยรวมมากที่สุดต่อการพัฒนาระบบบริการผู้ป่วยติดเตียงที่ยากไร้ รองลงมา คือ ภาคีการมีส่วนร่วม นโยบายระบบบริการผู้ป่วยติดเตียงที่ยากไร้ และนวัตกรรมบริการ ตามลำดับ 3) แนวทางการพัฒนาระบบบริการผู้ป่วยติดเตียงที่ยากไร้ คือ 3.1) การพัฒนาระบบบริการผู้ป่วยติดเตียงที่ยากไร้ควรครอบคลุมการเพิ่มการเข้าถึงบริการด้วยหน่วยบริการเคลื่อนที่และระบบข้อมูลกลาง 3.2) การประยุกต์ใช้เทคโนโลยีดิจิทัลและ IoT อย่างเหมาะสมกับบริบทชุมชน 3.3) การพัฒนาและถ่ายทอดแนวปฏิบัติมาตรฐานการดูแลที่ใช้งานได้จริง 3.4) การสนับสนุนนโยบายและงบประมาณอย่างต่อเนื่องและบูรณาการ และ 3.5) การส่งเสริมการมีส่วนร่วมของชุมชนและภาคีเครือข่ายทุกภาคส่วน เพื่อสร้างระบบบริการที่เป็นธรรม ครอบคลุม และยั่งยืน
