บทคัดย่อ

การวิจัยครั้งนี้เป็นการวิจัยเชิงคุณภาพแบบพรรณนา มีวัตถุประสงค์เพื่อบรรยายประสบการณ์การให้บริการดูแลสุขภาพผู้ป่วยที่บ้านของพยาบาลผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่น ผู้ให้ข้อมูล คือ พยาบาลวิชาชีพผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่นและมีประสบการณ์ในการให้บริการดูแลสุขภาพผู้ป่วยที่บ้านอย่างน้อย 2 ปี จากเขตบริการสุขภาพ 1, 9, 10, 11 และ 13 รวมจำนวนทั้งหมด 12 คน เก็บรวบรวมโดยการสัมภาษณ์เชิงลึก
การบันทึกเสียงขณะสัมภาษณ์ บันทึกภาคสนามร่วมกับการสังเกต เครื่องมือเก็บรวบรวมข้อมูล ประกอบด้วย ตัวผู้วิจัย แนวคำถามปลายเปิด แบบบันทึกภาคสนาม และเครื่องบันทึกเสียง วิเคราะห์ข้อมูลตามวิธีการวิเคราะห์แก่นสาระ และตรวจสอบความน่าเชื่อถือของข้อมูลตามเกณฑ์ของ Lincoln and Guba

ผลการวิจัยพบว่า การให้บริการดูแลสุขภาพผู้ป่วยที่บ้านของพยาบาลผู้ประกอบการคลินิกพยาบาลชุมชนอบอุ่น ประกอบด้วย 3 ประเด็นหลัก และ 9 ประเด็นย่อย ดังนี้ 1) การจัดบริการเยี่ยมบ้าน ประกอบด้วย 1.1) ลักษณะผู้ป่วยที่มีความจำเป็นให้บริการเยี่ยมบ้าน 1.2) การเข้าถึงรายชื่อผู้ป่วยที่ต้องเยี่ยมบ้าน 1.3) ช่วงเวลาการเยี่ยมบ้าน 2) ขั้นตอนการให้บริการ ประกอบด้วย 2.1) ตรวจสอบสิทธิและยืนยันตัวตนผู้ป่วย 2.2) ให้บริการพยาบาลตามอาการของโรค 2.3) ประเมินผลลัพธ์จากการเยี่ยมบ้าน 2.4) ลงระบบ E-Claim เบิกค่าบริการ และ 3) ปัญหาการเบิกจ่ายค่าบริการ ประกอบด้วย 3.1) การเรียกคืนเงินค่าบริการเยี่ยมบ้าน 3.2) ปัญหาการยืนยันตัวตนผู้ป่วยเยี่ยมบ้าน

ผลการวิจัยชี้ให้เห็นถึงความสำคัญของการให้บริการดูแลสุขภาพผู้ป่วยที่บ้านโดยพยาบาลผู้ประกอบคลินิกพยาบาลชุมชนอบอุ่น และปัญหาที่เกิดขึ้นในการให้บริการดูแลสุขภาพผู้ป่วยที่บ้าน ดังนั้น ควรมีการพัฒนารูปแบบการดูแลสุขภาพผู้ป่วยที่บ้านให้มีประสิทธิภาพมากขึ้น และมีการกำหนดขอบเขตการให้บริการดูแลสุขภาพผู้ป่วยที่บ้านจากหน่วยงานที่เกี่ยวข้องที่เหมือนกัน
