Predictors of Rapid Renal Function Decline in Patients with Diabetes and Hypertension in Chronic Kidney Disease Stages 3-4

Abstract

This predictive correlational study aimed to identify the predictive factors related to the rapid decline of glomerular filtration rate (GFR) in diabetic and hypertensive patients with chronic kidney disease (CKD) stages 3-4 at Phukamyao Hospital, Phayao Province. The study employed a predictive correlational design, focusing on personal factors, illness factors, health care behavior factors, and health literacy factors. The participants consisted of 155 diabetic and hypertensive patients. Data were analyzed using descriptive statistics, Pearson's correlation coefficient, and multiple regression analysis to identify predictive factors.

The findings revealed that most participants were female (58.10%), aged 60-65 years (23.9%), had primary education (86.50%), and had comorbidities (16.80%). Pearson's correlation coefficient analysis indicated that health literacy factors, including knowledge and overall health literacy level, were significantly related to the rapid decline of GFR (p < 0.05).  Multiple regression analysis showed that knowledge level (r = 0.372) and health literacy level (r = 0.045) could predict the rapid decline in GFR, accounting for 41.70% of the variance.  The results of this study can be used to develop an effective program to delay kidney failure in diabetic and hypertensive patients with CKD stages 3-4.
