The Effects of Skill Development Program for Village Health Volunteers on Physical Recovery of Post-Stroke Patient during Transitional Period after Hospital Discharge for Home Rehabilitation

การวิจัยกึ่งทดลอง แบบสองกลุ่มวัดก่อนหลังการทดลอง นี้มีวัตถุประสงค์เพื่อศึกษาผลของโปรแกรมพัฒนาทักษะในการช่วยฟื้นฟูสภาวะทางกายของผู้ป่วยโรคหลอดเลือดสมอง ในระยะเปลี่ยนผ่านหลังการจำหน่ายจากโรงพยาบาล เพื่อกลับไปฟื้นฟูสภาพที่บ้านของอาสาสมัครสาธารณสุขประจำหมู่บ้าน (อสม.) คัดเลือกกลุ่มตัวอย่างโดยการสุ่มอย่างง่าย จากอสม. ของอำเภอสองพี่น้อง จังหวัดสุพรรณบุรี ได้กลุ่มทดลอง 28 ราย กลุ่มเปรียบเทียบ 28 ราย กลุ่มทดลองได้รับโปรแกรมการพัฒนาทักษะในการช่วยฟื้นฟูสภาวะทางกายที่ประยุกต์ใช้ทฤษฎีการรับรู้ความสามารถของตนเองในการจัดกิจกรรม ซึ่งกิจกรรมประกอบด้วย การให้ความรู้ เรียนรู้จากตัวแบบ สาธิตการฟื้นฟูสภาพและสาธิตย้อนกลับ เป็นเวลา 2 สัปดาห์ ส่วนกลุ่มเปรียบเทียบจะได้รับการจัดกิจกรรมการพัฒนาทักษะดังกล่าว หลังเสร็จสิ้นโปรแกรม เก็บข้อมูลโดยตอบแบบสอบถามด้วยตนเอง วัดผลหลังการทดลอง และระยะติดตามผล 2 สัปดาห์หลังการทดลอง วิเคราะห์ข้อมูลโดยสถิติ Chi square, Independent t-test และ Repeated Measure ANOVA
ผลการวิจัย พบว่า หลังการทดลองและระยะติดตามผล กลุ่มทดลองมีคะแนนเฉลี่ยทักษะการฟื้นฟูสภาวะทางกายผู้ป่วยโรคหลอดเลือดสมองด้านการป้องกันแผลกดทับ การป้องกันพลัดตกหกล้ม การเคลื่อนไหว เคลื่อนย้ายและพลิกตะแคงตัว การป้องกันข้อยึดติด การฝึกการทรงตัว และการส่งเสริมความแข็งแรงของกล้ามเนื้อ สูงกว่าก่อนการทดลองและสูงกว่ากลุ่มเปรียบเทียบอย่างมีนัยสำคัญทางสถิติ (p-value < .05) โปรแกรมดังกล่าวสามารถนำไปใช้ในการพัฒนา อสม.ให้มีความสามารถและความมั่นใจในการช่วยฟื้นฟูสภาวะทางกายให้กับผู้ป่วยโรคหลอดเลือดสมองในชุมชน เพื่อช่วยให้กลับฟื้นคืนสู่สภาวะที่ใกล้เคียงภาวะปกติมากที่สุด ลดความพิการ และภาวะพึ่งพิง
