ความสัมพันธ์ระหว่างการตรวจประเมินคุณภาพการบันทึกเวชระเบียน กับการตรวจสอบการสรุปและให้รหัสทางการแพทย์ในประเทศไทย

บทคัดย่อ
การวิจัยเชิงพรรณนาแบบศึกษาย้อนหลังนี้ มีวัตถุประสงค์เพื่อศึกษาความสัมพันธ์ระหว่างผลการตรวจประเมินคุณภาพการบันทึกเวชระเบียนกับผลการตรวจสอบการสรุปและให้รหัสทางการแพทย์ เก็บรวบรวมข้อมูลย้อนหลังจากผลการตรวจประเมินคุณภาพการบันทึกเวชระเบียนผู้ป่วยใน และผลการตรวจสอบการสรุปและให้รหัสทางการแพทย์ โดยสำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.) ระหว่างวันที่ 1 ตุลาคม พ.ศ. 2561 ถึงวันที่ 30 กันยายน พ.ศ. 2562 ของโรงพยาบาลที่เป็นหน่วยบริการในระบบหลักประกันสุขภาพแห่งชาติ จำนวน 910 แห่ง ซึ่งได้จากการสุ่มอย่างเป็นระบบ วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและการวิเคราะห์สัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน ผลการวิจัยพบว่า
1. ผลการตรวจประเมินคุณภาพการบันทึกเวชระเบียน หัวเรื่องที่มีคะแนนเฉลี่ยสูงที่สุดคือ บันทึกสรุปการจำหน่าย (อื่นๆ) (M = 8.31, SD = 0.66) และต่ำสุดในหัวเรื่องบันทึกการปรึกษาโรคระหว่างแผนกหรือกลุ่มงานของแพทย์ (M = 5.76, SD = 1.78) และมีเปอร์เซ็นต์เฉลี่ยเท่ากับ 80.57 (SD = 8.69) ผลการประเมินความถูกต้องการสรุปการวินิจฉัยโรคและหัตถการของแพทย์พบว่า ผู้ประเมินไม่มีความเห็นที่แตกต่างเกี่ยวกับการสรุปการวินิจฉัยและการสรุปหัตถการในสรุปการจำหน่ายของแพทย์ คิดเป็นร้อยละเฉลี่ย 43.88 (SD = 12.91) และการประเมินความถูกต้องการให้รหัสการวินิจฉัยโรคและหัตถการพบว่า ผู้ประเมินมีความเห็นที่ตรงกันกับผู้ให้รหัส คิดเป็นร้อยละเฉลี่ย 70.98 (SD = 15.09)
2. เปอร์เซ็นต์เฉลี่ยของผลการตรวจประเมินการบันทึกเวชระเบียนมีความสัมพันธ์ทางบวกในระดับต่ำกับผลการประเมินความถูกต้องซึ่งผู้ประเมินไม่มีความเห็นที่แตกต่างเกี่ยวกับการสรุปการวินิจฉัยและการสรุปหัตถการในบันทึกการสรุปการจำหน่ายของแพทย์ และผลการประเมินซึ่งผู้ประเมินไม่มีความเห็นที่แตกต่างเกี่ยวกับการให้รหัสโรคและหัตถการของผู้ให้รหัส อย่างมีนัยสำคัญทางสถิติที่ระดับ .01 (r = .097, .131)
โรงพยาบาลควรมีการพัฒนาให้ความรู้เพิ่มทักษะเกี่ยวกับการตรวจประเมินคุณภาพการบันทึกเวชระเบียน การตรวจประเมินคุณภาพการสรุปและให้รหัสทางการแพทย์แก่บุคลากร และดำเนินการตรวจสอบภายในเป็นประจำอย่างต่อเนื่องโดยการสนับสนุนเชิงนโยบายและเทคโนโลยีสารสนเทศ
