DEVELOPMENT OF A CLINICAL NURSING PRACTICE GUIDELINE (CNPG) FOR NEWBORN INFANTS WITH CENTRAL VENOUS CATHETERIZATION

Abstract

This research and development project aimed to 1.Study and develop nursing practice guidelines for newborns to prevent complications from central venous catheter (CVC) insertion. 2.Evaluate the outcomes of implementing these nursing practice guidelines for newborns with CVCs. 3.Assess the outcomes of using an innovative patient assessment tool for those with CVCs in the neonatal ward at Khon Kaen Hospital. The study sample consisted of 10 newborn patients who underwent CVC insertion and 18 professional nurses responsible for their care. This was a developmental research study using Soukup’s evidence-based practice framework, which includes four phases 1.Identifying clinical problems. 2.Searching for evidence-based practices 3. Developing and piloting nursing practice guidelines. 4.Implementing the improved and refined guidelines in clinical settings. The study was conducted from March 16, 2023, to March 30, 2024. The results showed that there were no bloodstream infections related to CVC insertion. However, complications such as catheter occlusion and dislodgement were observed. The complication rate decreased by 10% compared to the previous year, and no adverse events above severity level C were reported. Most professional nurses expressed a high level of overall satisfaction (𝑥̅ = 5, S.D. = 2.6). The innovative CVC patient assessment form proved to be a valuable tool for early detection, allowing nurses to assess patients promptly before harm occurred. This innovation contributed to reducing the severity of adverse events, shortening patient hospital stays, and lowering hospital expenses.
