Development of a Case Management System for the Chronic Obstructive Pulmonary Disease at Nakornping Hospital

Abstract

The objective of this action research was to develop a caring system for the chronic obstructive pulmonarydisease based on case management. The study’s participants included of registered nurses who worked in the unit, an interdisciplinary health care team, and those who worked in the primary care units (PCUs), chronic obstructive pulmonary disease (COPD) patients and relativer or care givers. This study was conducted from September 2008 to October 2010. Data was analyzed using descriptive statistics and content analysis. The fnding showed that the care management system of COPD using collaborative and participation from multidisciplinary, family and community from hospital to PCUs. After using this system, patients were able to self care, length of stay was reduced to average from 10 days shortened to 6 days. Readmission rate was reduced from 14.41% to 7.15%. The levels of patients’ satisfaction and nurses’ satisfaction were in the high level.
