Case management is a concept of continuity of care that focuses on health services for patients with complex problems and needs, such as patients with chronic illness. The purpose of case management is to control medical expanses under the optimal quality of care, to provide care that responds to specific problems and needs of the patient, and to develop a continuity of care through cooperation, care management, communication, and referral the patient to a proper health facility. The outcomes of case management for the patient with chronic illness are to control the symptoms, to prevent complications, to delay the progression of the illness, and to improve the patient’s quality of life. The outcome of case management for healthcare system is to break even costs under optimum health. This article presents a concept of case management and a role of the nurse in case management for patients with chronic illness. Moreover, a case study demonstrating the use of case management in patients with chronic obstructive pulmonary disease is presented to help nurses understand the concept and be able to use it to improve nursing practice.
