บทคัดย่อ

The objective of this independent study was to develop the continuity of care service system in the family with stroke patients. The study was divided into three phases. Phase I aimed to study the circumstance of continuity care from clients and health care provider’s perspective. Phase II aimed to develop the continuity of care service system in the family of stroke patients. Phase III aimed to evaluate the continuing care and analyze data. Content analysis was used to analyze the qualitative data and descriptive statistics was applied to describe the quantitative data. The study found that the clients and health care providers reflected their view based on the types of continuing care divided into five categories as follows: 1) Information continuity 2) Management continuity 3) Relational continuity 4) Community continuity of care 5) continuity of care with the health care team. The results from this study led to the development of continuing care services system in family with stroke patients from the rehabilitation phase in the hospital to the transitional phase and also the continuous care in the community. There were also the improvement in the score of health care provider’s service and the client’s satisfaction after the study when compared with pre-study.
