Development of discharge planning model for cerebrovascular disease using Independent room

This action research study aimed to develop the discharge planning in patient care process. The study consisted of three phases: 1) preparation phase included situation analysis and care planning; 2) implementation phase; After the implementation of developed program, data were collected from the case studies using data collection forms, self-care assessment forms, self-confidence forms and 3) evaluation phase;The results showed that discharge planning models could improve the ability and confidence of caregiver in stroke patient care. In conclusion, discharge planning model could improve care givers knowledge and confidence in rehabilitation care. That model should be tailored for individual and should be completed early in admission and re-evaluated periodically depend on patient’s condition.Downloads
