ปัจจัยทำนายความเครียดหลังคลอดในโรงพยาบาลชุมแพ จังหวัดขอนแก่น

การวิจัยเชิงพรรณนาแบบทำนาย เพื่อศึกษาปัจจัยทำนายความเครียดของมารดาหลังคลอดใน 48-72 ชั่วโมงหลังคลอด กลุ่มตัวอย่างเป็นมารดาหลังคลอดในโรงพยาบาลชุมแพ จังหวัดขอนแก่น จำนวน 306 คน ดำเนินการวิจัยตั้งแต่ 30 กรกฎาคมถึง 30 ตุลาคม 2566 เครื่องมือวิจัยที่ใช้เป็นแบบสอบถามข้อมูลทั่วไป ได้ค่าดัชนีความตรงเชิงเนื้อหา เท่ากับ 1 และแบบประเมินความเครียด ST5 ได้ค่าสัมประสิทธิ์ แอลฟาครอนบาค เท่ากับ 0.8 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณา สถิติไคสแควร์และการวิเคราะห์ ถดถอยโลจีสติกแบบ 2 กลุ่ม
ผลการวิจัย พบความเครียดของมารดาหลังคลอด ระดับปานกลางถึงระดับมาก ร้อยละ 15.7 แยกเป็นระดับปานกลาง ร้อยละ 13.7 ระดับมาก ร้อยละ 2.0 ปัจจัยทำนายความเครียดหลังคลอด ได้แก่ มีประวัติเคยถูกทำร้ายทางจิตใจ (=18.4, p=0.002) มีภาวะแทรกซ้อนหลังคลอด (
=17.4, p<0.001) ความสัมพันธ์ในครอบครัวไม่ดี (
=5.4, p<0.001) ระยะเวลาที่ตั้งใจเลี้ยงลูกด้วยนมแม่ น้อยกว่า 6 เดือน (
=4.3, p=0.002) และมีความวิตกกังวลระหว่างตั้งครรภ์ (
=3.1, p=0.011) โดยสามารถอธิบายความแปรปรวนของความเครียดหลังคลอด ได้ร้อยละ 42 (
=0.42, p<0.001)
สรุป ควรคัดกรองและประเมินความเครียดมารดาหลังคลอดทุกรายก่อนการจำหน่าย เพื่อให้มารดาได้รับการดูแล ช่วยเหลือที่รวดเร็ว ป้องกันและเฝ้าระวังการเกิดความเครียดหลังคลอดที่รุนแรง
