ผลของโปรแกรมดูแลผู้สูงอายุข้อเข่าเสื่อมโดยการมีส่วนร่วมของชุมชน ด้วยการนวดแผนไทย ร่วมกับการพอกสมุนไพรและการดูแลตนเอง ต่อระดับความรุนแรงข้อเข่าเสื่อมของผู้สูงอายุ ในโรงพยาบาลส่งเสริมสุขภาพตำบล เขตอำเภอเมือง จังหวัดพิษณุโลก

การวิจัยนี้เป็นการวิจัยกึ่งทดลองแบบวัดก่อนหลังการทดลอง เพื่อศึกษาผลของโปรแกรมดูแลผู้สูงอายุ ข้อเข่าเสื่อมโดยการมีส่วนร่วมของชุมชนด้วยการนวดแผนไทยร่วมกับการพอกสมุนไพรและการดูแลตนเองต่อระดับความรุนแรงข้อเข่าเสื่อมในผู้สูงอายุที่โรงพยาบาลส่งเสริมสุขภาพตำบลเขตอำเภอเมือง จังหวัดพิษณุโลก
กลุ่มตัวอย่างเป็นผู้สูงอายุข้อเข่าเสื่อมในโรงพยาบาลส่งเสริมสุขภาพตำบลอำเภอเมือง จังหวัดพิษณุโลก จำนวน 60 คน สุ่มตัวอย่างด้วยวิธีเลือกแบบเจาะจง ตามคุณสมบัติที่กำหนด แบ่งเป็นกลุ่มทดลองที่ 1 กลุ่มทดลองที่ 2 และกลุ่มควบคุมกลุ่มๆ ละ 20 คน ที่มีลักษณะคล้ายคลึงในเรื่องเพศ อายุ ภาวะข้อเข่าเสื่อม การรักษาข้อเข่าเสื่อมและระดับความรุนแรงข้อเข่าเสื่อม กลุ่มทดลองที่ 1 ได้รับการนวดแผนไทยจากหมอนวดชุมชนที่ผ่านการทดสอบหลังอบรมฟื้นฟูเทคนิคการนวดแผนไทยสำหรับบำบัดข้อเข่าเสื่อม (ร่วมกับรับประทานยาแผนปัจจุบัน) กลุ่มทดลองที่ 2 ได้รับการนวดแผนไทยจากหมอนวดชุมชนที่ผ่านการทดสอบหลังอบรมฟื้นฟูเทคนิคการนวดแผนไทยบำบัดข้อเข่าเสื่อม (ร่วมกับการพอกสมุนไพร) และผู้วิจัยสนับสนุนการให้ความรู้ และการสร้างสิ่งแวดล้อม ที่เหมาะสม เพื่อให้เกิดการดูแลตนเองเพื่อลดปัจจัยทำให้ข้อเข่าเสื่อม ขณะที่กลุ่มเปรียบเทียบได้รับการดูแลตามปกติ รวบรวมข้อมูลระหว่างมิถุนายน-พฤศจิกายน 2564 เครื่องมือวิจัย ประกอบด้วย โปรแกรมดูแลผู้สูงอายุข้อเข่าเสื่อมโดยการมีส่วนร่วมของชุมชนด้วยการนวดแผนไทยร่วมกับการพอกสมุนไพรและการดูแลตนเองต่อระดับความรุนแรงข้อเข่าเสื่อมฯ แบบประเมินคัดกรองภาวะข้อเข่าเสื่อม และแบบประเมินระดับความรุนแรงข้อเข่าเสื่อมวิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา การแจกแจงความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติเชิงอนุมาน repeated measures ANOVA
ผลการวิจัยพบว่า ผู้สูงอายุที่มีภาวะข้อเข่าเสื่อมหลังได้รับการนวดแผนไทยร่วมกับการพอกสมุนไพรมีคะแนนเฉลี่ยระดับความรุนแรงข้อเข่าเสื่อมดีขึ้นกว่าผู้สูงอายุหลังได้รับการนวดแผนไทยร่วมกับการรับประทานยาแผนปัจจุบันและผู้สูงอายุที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติ (F=63.71, p<.01) ดังนั้น จึงสรุปได้ว่าโปรแกรมมีประสิทธิภาพในการลดระดับความรุนแรงของข้อเข่าเสื่อมได้ สามารถประยุกต์ใช้ในการดูแลผู้สูงอายุข้อเข่าเสื่อมเพื่อส่งเสริมให้ชุมชนดูแลตนเองได้และลดค่าใช้จ่าย การพัฒนารูปแบบต่อไปควรเพิ่มหุ้นส่วนสุขภาพที่หลากหลายเพื่อเพิ่มประสิทธิภาพยิ่งขึ้น
