The effects of the older adult care program for knee osteoarthritis by community participation using traditional Thai massage combined with herbal wraps and self-care on severity level of knee osteoarthritis in a Tambon Health Promoting Hospital, Muang District, Phitsanulok Province

Abstract

This quasi-experimental research (pretest and posttest design) aimed to study the effects of the older adult care program for knee osteoarthritis using community participation with traditional Thai massage combined with herbal wraps and self-care on knee osteoarthritis severity among older adults in a Tambon Health Promoting Hospital, Muang District, Phitsanulok Province.


Sample comprised 60 older adults with knee osteoarthritis at a Tambon Health Promoting Hospital, Muang District, Phitsanulok Province. Purposive sampling was used to recruit participants according to inclusion criteria. Sample were then divided into the experimental group I, the experimental group II, or the control group. Each group consisted 20 people. All three groups were similar, in terms of gender, age, knee osteoarthritis condition, knee osteoarthritis treatment, and knee osteoarthritis severity. The experimental group I received traditional Thai massage from Thai massage therapist who passed posttest after the refreshment course in traditional Thai massage technique for osteoarthritis treatment (in combination with taking modern medicine). The experimental group II received traditional Thai massage from Thai massage therapist who passed posttest after the refreshment course in traditional Thai massage technique for osteoarthritis treatment (in combination with herbal wraps). The researcher provided both experiment groups with knowledge support and suitable environment for self-care to reduce factors contributing to osteoarthritis, whereas the comparison group received usual care. Data collection was carried out from June to November 2021. Research instruments included the older adult care program for knee osteoarthritis using community participation with traditional Thai massage combined with herbal wraps and self-care, osteoarthritis assessment questionnaire, and osteoarthritis severity assessment questionnaire. Data analysis was performed using descriptive statistics; including frequency, distribution, percentage, mean, standard deviation; and inferential statistics, including repeated measures ANOVA.


The results showed that after the experiment, older people with knee osteoarthritis who received traditional Thai massage combined with herbal wraps, had statistically significant better severity level of knee osteoarthritis than those received traditional Thai massage combined with taking modern medicines, and better than those in the comparison group (F=63.71, p<.01). It is suggested that the program was effective in reducing knee osteoarthritis severity. It should be applied in caring for older people with knee osteoarthritis to promote community to take care of themselves and reduce expenses. Various health partners should be added for further
development of the model in order to increase efficiency.
