The purposes of this research and development were: 1) to develop a nursing form information documented in the patients with severe traumatic brain injury in 48 hours. 2) Satisfaction on a nursing form information documented by the registry nurse. The studied consisted of 4 steps; 1) problems and needs study 2) nursing form information documentation designed and development 3) nursing form information documented tired 4) evaluation and improvement of nursing form that the evident base practices by Soukup (2000). The sample were 15 registered nurses and 10 traumatic brain injury patients in 48 hours after admit. The tools consisted of nursing form information documented (CVI 0.80) and the satisfaction evaluation. Result 100 % nurses participated satisfaction on a nursing information documentation and 100 % on apply to record nursing intervention because develop to total nursing care. Conclusion 1) the nursing form information documented must be trained and monitor information outcome. 2) data analyzed for improvement.
