การดูแลครอบครัวที่มีสมาชิกเจ็บปวยเรื้อรัง เปนภาวะที่ตองการการดูแลเปนระยะเวลายาวนาน รักษา ไมหายขาด ตองการการดูแลควบคุมอาการของโรคและปองกันไมใหเกิดภาวะแทรกซอน จําเปนตองไดรับการ ดูแลตอเนื่อง แตอยางไรก็ตามแนวคิดการดูแลตอเนื่องเปนแนวคิดที่มีการใหความหมายนิยามที่กวาง เปลี่ยนแปลง ไปตามกาลเวลาและบริบท และคาบเกี่ยวกับแนวคิดอื่นๆ การนําแนวคิดการดูแลตอเนื่องไปใช พยาบาลตองมี ความเขาใจเกี่ยวกับแนวคิดการดูแลตอเนื่อง เพื่อที่จะประยุกตใชแนวคิดการดูแลตอเนื่องในการพยาบาลครอบครัว ที่มีสมาชิกเจ็บปวยเรื้อรัง สงผลใหผูปวยและครอบครัวมีคุณภาพชีวิตที่ดีขึ้น บทความจะแบงเนื้อหาเปน 2 ตอน บทความนี้เปนตอนที่ 1 แนวคิดการดูแลตอเนื่อง ประกอบดวย ความหมาย ชนิด ผลลัพธและการประเมินการดูแล ตอเนื่อง และตอนที่ 2 การประยุกตใชแนวคิดการดูแลตอเนื่องในการพยาบาลครอบครัวทีมีสมาชิกเจ็บปวยเรื้อรัง ซึ่งจะนําเสนอตอไป

Abstract

Caring for family with chronic illness persons, which need a long time of caring and the disease is not usually cured, requires the continuity of care to prevent and control symptoms and also prevent complications. However, continuity of care concept is defined broadly, has changed over time due to contextual factors and overlap with relating concepts. Thus, nurses should have deeply understanding this concept in order to appropriately apply for caring the family with chronic illness persons and then can improve patients’ and family’s quality of life and also quality of care. The article is divided into 2 parts. This part 1 continuity of care concept article will be presented the definition, type, outcomes and measurement of continuing of care. The application of this concept will be presented later in part 2.
