บทคัดย่อ

การจัดการรายกรณี เป็นกระบวนการในการดูแลผู้ป่วย เพื่อเพิ่มคุณภาพการดูแลที่เน้นผลลัพธ์ด้านสุขภาพ ซึ่งต้องอาศัยทรัพยากรบุคคลด้านสาธารณสุข แหล่งประโยชน์ในชุมชน เครือข่ายในการจัดการระบบการทำงานแบบบูรณาการ พยาบาลในบทบาทของผู้จัดการรายกรณีเป็นองค์ประกอบหนึ่งที่สำคัญสามารถสนับสนุนการเข้าถึงการบริการสุขภาพอย่างเป็นรูปธรรมในชุมชน โดยใช้เครื่องมือ คือ แผนการดูแลของทีมสุขภาพที่พัฒนาขึ้นมา และสามารถประยุกต์กระบวนการพยาบาลมาใช้ในการจัดการรายกรณี โดยใช้ในการค้นหาคัดกรอง ประเมินปัญหาความต้องการ วางแผนการทำงาน ดำเนินการปฏิบัติประเมินผล และมีการกำกับกระบวนการจัดการรายกรณีที่เหมาะสมกับความต้องการของผู้สูงอายุที่มีภาวะพึ่งพาในชุมชนผู้ดูแล และครอบครัว ซึ่งภาวะพึ่งพามักจะเกี่ยวพันกับภาวะทุพพลภาพเป็นปัจจัยหลัก การประเมินภาวะพึ่งพาในผู้สูงอายุ จึงถือเป็นสมรรถนะที่จำเป็นสำหรับพยาบาลผู้จัดการรายกรณี

Abstract

Elderly dependent care is an important care for community nurse that will have to including case management competency. Case management is a caring method to enhance the quality of care that is often used with elderly in community who is disable or needed help. This method is required integrated management systems from providers and community resources. Community nurse plays a crucial role to enhance healthcare assessing in community by developing strategies for health plans. Nursing process is an important tool to adapt and modify in case management plan that consists screening, accessing, nursing diagnosis, planning, and evaluation. However, the best care management has to appropriate to patient, caregiver, family, and community needs.
