Development of Clinical Nursing Practice Guidelines for the Prevention of Infection in Patients with Solid Tumors after Receiving Chemotherapy

This participatory action research aimed to develop and evaluate the implementation of clinical nursing practice guidelines for the prevention of infection in patients with solid tumors receiving chemotherapy. The study was guided by Soukup’s evidence-based practice model, with staff nurses participating in the guideline development process. The participants comprised 19 registered nurses and 38 patients, who were assigned to a control group (n = 19) and an experimental group (n = 19). The research instruments included the clinical nursing practice guidelines, a self-care handbook, a knowledge questionnaire, a self-care behavior questionnaire, and a feasibility assessment form. The instruments demonstrated good content validity (CVI = .90 - 1.00) and reliability coefficients of .88. Data were analyzed using descriptive statistics, independent t-test, paired t-test, chi-square test, and Fisher’s exact test.
The findings showed that the guidelines covered nursing care before, during, and after chemotherapy. Nurses demonstrated a 97.58% compliance rate, and the overall feasibility of guideline implementation was rated high (Mean = 2.75, SD = .41). Compared with the control group, the experimental group showed significantly higher knowledge scores (MD = 3.16, 95%CI 2.12 - 4.20, t = 6.15, p-value < .001) and self-care behavior scores (MD = 5.58, 95%CI 4.61 - 6.55, t = 11.62, p-value < .001). No infection occurred in the experimental group, whereas two patients in the control group developed infections. Nurses reported that the guidelines were clear and feasible for routine clinical practice.
