The Development and Effects of Clinical Nursing Practice Guideline: Central Venous Catheters Associated Complication Prevention

Abstract

This research and development study aimed to develop a clinical nursing practice guideline (CNPG) to prevent complications associated with central venous catheter (CVC), The effects of the CNPG on knowledge, nursing practice skills, satisfaction and CVC-related complications were evaluated. The participants were 36 registered nurses and 14 patients. Research instruments included a knowledge test and a nursing practice skills assessment form (Kuder-Richardson reliability at .85 and .87, respectively), the CNPG and a satisfaction questionnaire. (content validity index at .85 to .88). Data were analyzed using descriptive statistics, paired samples t-test and one-way repeated measures ANOVA.

The results revealed that the evidence-based nursing practice guideline comprised 18 nursing care activities. Following implementation of the CNPG, the mean knowledge score increased significantly (t = -15.05, p-value < .001). The mean nursing practice skill scores at 1, 4, and 12 weeks were significantly different (F (2.88, 100.66) = 82.18, p-value < .001). Overall satisfaction with the guideline was at the highest. Overall satisfaction was at the highest level (Mean = 4.72, SD = .45). Regarding complications, no catheter-related infection or bleeding was observed; catheter occlusion occurred at a rate of 2.42 per 1,000 catheter-days. These findings indicate that the evidence-based CNPG is effective and should be promoted with monitoring of adherence, may improve the quality of CVC nursing care in accordance with professional standards.
